How old is your husband?
His elevated PSA could be caused by other non cancerous prostate conditions and infections, as could his knobbly prostate. With a PSA of 18, it's right that he's been referred to urology. I suspect they might advise that he has an MRI, which is much more accurate than a DRE and will give a much clearer picture of his condition.
