Chemo or Enzalutimide

Hi Allison, sorry to hear John is unwell. I assume that when they gave you anti biotics for the suspected UTI they also sent if off for analysis. That way it can tell them which anit biotic would be effective. I know this from bitter experience as a UTI with me got entrenched because I was initially on the wrong regime. If you are on the right anti biotics then you should see improvements in the colour of the urine after a few days. Mine goes cloudy and at its worst reddy but clears quite clearly on anit biotics. The symptoms can last longer including discomfort on the penis and need to pee urgently. If it is a kidney infection they should have picked that up from the tests too. If they did not test your urine that will be a question to ask.

Isn't PSA confusing? I have never had PSA that low but have had no pain so far. I waited until PSA a was 42 before getting Casodex which appears to be working for me for now, I know Chemo drugs are next and my uro is recommending aberiterone. I am awaiting an appointment with an onco to check out their view.

Sorry you are is this uncertain position it must be very worrying. You know we are all here thinking of you, I do hope you get answers very soon,

Hi Allison,

I'm sorry John is not feeling well.  I can't really help with any useful advice but I just wanted to send you and John my best wishes and hope everything is sorted soon.

All the best from here in Reading to you in Swindon,

Steve

Allison my lovely friend

why did you not tell me that John had been so poorly when I saw you in March? I know you have also had all sorts of issues to deal with and I know you worry about me and the stuff that is going on in my life but you also need the love and hugs and need to reach out for them without hesitation.

If John has any kind of UTI it will lay him low, getting the right anti biotics and treatment is critical so lab testing his sample as Paul has said is very important.

I hate to say this but you know I bang on about SCC all the time for the advanced mets guys, when did John last have an MRI to check that the RT he had has shrunk the mets away from any critical impact?. If John can still pass both urine and stools ok then that is a positive. Also if he is still walking ok and not too weak in the legs the Tirednees may just be slow progression and perhaps overdoing it with that gorgeous grand baby of yours. I dont think in John's case PSA is the best indicator it is low in comparison with some and reltively slow moving so not really something you can track progress on. I think his general well being is more important so if he feels bad then you should be asking questions of his medical team. IMHO it may be that it is time for a change of treatment to Abby or Enza or possibly chemo just to knock the cancer back once again. What you do know after 4 years on this journey is that John does generally respond well to new treatment.

I am thnking of you and will try to get to see you when I get home and after Jen's funeral.

I would be concerned about the kidneys too, Allison. Did they send off his urine sample for detailed analysis, as Paul mentioned? If not, perhaps you could ask for that to be done this week? Also, I might be wrong but think it is a while since his last scan so perhaps time to request one of those as well?

You have had such a busy time and the excitement of baby D coming into your lives - perhaps John is doing more moving around than he did before, without either of you really noticing. Bending, lifting, pushing will all be a strain on that hip but the movements are unconscious - if John were going to the gym and doing more exercise, you wouldn't be surprised at the fatigue, aches & pains but because it is princess related rather than gym related, it is imperceptible.

Stay brave and ask for more tests, lovely friend x

Hi Allison I am sorry you are going through such a tough time. Other replies have a given good sound advice. I can't answer regarding uti as we luckily haven't had that one. I know people cope differently on treatments but my oh has responded well to docetaxel enzalutamide and now cabazataxel. They have all helped with pain and reduced the psa for a reasonable amount of time. You have always replied to my post so positively so please take heart and hopefully whichever treatment is next will give good positive results. Xxx

Hi Alison,

Sorry to hear of your frustrations and worries. Empathise greatly with your situation and my heart goes out to you.

I agree with others that you should send a urine sample to be tested for appropriate antibiotic that's hoping it's a uti not the cancer. Am hoping it may just be that the PSA has risen due to uti rather than cancer being active. Maybe GP could order blood tests to check renal function may help indicate what's happening re kidneys.

HopING John is feeling better soon and gp helps you tomorrow. Fingers crossed for a simple solution.

Hi friends

The sample was sent off on Wednesday for testing but for what exactly I don't know, assume the type of infection rather than anything else. He's still the same today, pain is still in side and pain higher up in the back too. It could be the fact that he is picking up the baby and having lots of cuddles, but that wouldn't account for him feeling physically so low, in fact the opposite really.

As the holidays will be over tomorrow we will do something about it, I'm thinking phone the nurse at the GP in case the results are back (doubtful given the holidays) and try to speak to the specialist nurse at the hospital for more advice.. Of course, it could just be muscular, but and its a big but for me, John never complains - I didn't know that his Gall bladder and bile duct infection gave rise to pain at level 10 nor the Xmas and New Year pain, again a 10, until he had had that for some days. He simply doesn't mention it, so the fact that he is telling me every day that his back hurts as does his side, makes me worry.

We have had the family round today and had a lovely day, so that's a good thing.

Let's see what tomorrow brings. I don't expect any urgent action, even with severe pain at the New Yer we had to wait 19 days to see the oncology team.

Sorry hon, I think the opposite could be true. The low mood could be worry about the increasing pain and sadness at the prospect that one day he will have to leave his precious grand-daughter - you are such a close family but now he has a new and overwhelming need to stay around for as long as possible.

Hi Allison,

They are such a worry these men of ours. I have everything crossed that it is a UTI and as Paul has said they can be hard to shift, sometimes a different type of Aunty Bs is needed and it can take longer to see results but I know your concerns any worries can and do enter in to realms of panic very quickly in our situation.

A UTI can make you feel very low, and give the back ache that John is describing I won't give any advice on who or where to go because you are a PRO at this , been there done this and got several T shirts.

So just to say much love winging it's way and Stay strong.

Hi Allison,

Any concerns I would go down the get it checked and get some answers from the medics. Nothing worse than being worried and not knowing. You are both in my thoughts, and fingers crossed it is a UTI .

Well we are still waiting for results but John seems to be a little better today. He still has to rest for most of the day and is shockingly breathless (that's standard now for many months). Quick question, does anyone else have the shakes? He has been shaky in his hands for a fair while and it's getting worse, I'm at a loss to say what's causing it. Any thoughts?

Yes Lyn that has occurred to me too, there is a feeling of bittersweet sadness in the joy and John has said a few times that he feels so angry about how he had hoped his retirement would pan out and how it has turned out. He often tells me that the illness has taken its toll, it has on me too.

I was talking to our HR lady today (she's lovely) her hubby had a tumour in his throat and is now "cured" after traumatic surgery, RT and chemo. He is half the man he was, no memory, personality change, very slow, in and out of hospital for life threatening pneumonia incidents (caused by uncontrolled vomiting and inhaling, poor man) and monthly procedures to keep his oesophageal open, all of which are hideous. They have three children, the youngest is eleven. Compared to that we are very happy indeed, count your blessings and all that. I'm pleased I had a chat with her, I was definately going down mentally but sharing our pain was cathartic, we truly understand each other. The inevitable exhaustion involved when you care for someone, the reality of long term incurable Illness,, the constant near panic at each new symptom, afraid to look into the future for what we might see, all of that and much, much more. We understand each other here too and that's why I am so grateful for your replies and support. It really does help to share and get advice here. I have some perspective back today, thank heavens.

I'll let you all know what happens with the kidney pain.

Lots of love

Allison xx
