Seems to be a bit out of the blue as your bio shows your next appointment at Kings was to be on 17th July. You will doubtless have been told what to expect and how Nanoknife (Irreversible electroporation) works . It is considered very much an experimental treatment and although is promising, long term outcomes will take time before it's success can be determined.

Hope it goes well for you and like others would be interested in your immediate and ongoing experience.

Yes that was my next follow up originally to see [Doctors name removed by Moderator] at KCH for Prostate Removal meeting. But I got a call at the the start of April to see if I was interested in Nanoknife and had the consultation 18th April. For the original July meeting I was told I need to lose a bunch of weight to be considered for Prostate Removal Surgery so 2 days after my diagnosis I started a Weight Loss Challenge. So far lost 22lbs :) from 312lbs. Been a kick up the arse to lose weight.

8th May 2017


At KCH today for Pre-Admission checks plus as mentioned by LynEyre I'll ask about a PSA Test. 


Operation is the same as my Template Biopsy so hopefully recovery is the same. I had no pain after my Biopsy just 3 Urine Infections. Going in on Wednesday 10th May for Day Surgery but told I will stay overnight. With Electricity being fired at my Prostate I'm wondering if I'll wake up to pain this time. Well done on the weight loss challenge - brilliant

My husband is 6 weeks post FLA and his procedure was also much like his mapping biopsy except no general.

Side effects broadly similar to post biopsy except for a bout of retention after catheter removal.

Irreversible Electroporation (Nanoknife) in Ablation of Prostate Cancer. May 10th operation update: 


Went into Kings College Hospital at 12:30pm for Day Surgery. Team from Amsterdam were there to assist the 2 operations for myself and another chap. Procedure is similar to having a Template Biopsy with one difference. On meeting the Urologist prior to operation I was told that I would need a catheter due to Prostate swelling as a precaution for 2 days.


Having a chat with the Anesthetist prior to the op I asked to make sure the breathing tube is removed before I wake up. On my Biopsy I was awoke with the tube in my throat and was very stressful waking up. He said he would do his best. 


 


In theater: Had a joke with the Anesthetist as he said "ok ready to go under" I replied "Back to the Future Doc !" lol 


Post Op: I woke up with no breathing tube :) but I was in a LOT of pain. "OK John on a 1-10 with 10 being the worst pain please tell me your score", "yeah 20!" I then got topped up 2 or 3 times with morphine until they said that they could not give me any more. As I became more awake I had the feeling I was busting to urinate. I started to try and stand up and nurses tried to stop me "John your weak stay in bed you can just pee in the bag". I just needed to stand up and pee and proceeded to get out of bed holding onto a male nurse. I then urinated in the bag. This when I realized the excruciating pain was not from the procedure but from the catheter. They told me to sit in the chair but as I sat the pain got worse so I stood up and turned the chair around to lean on. I was sweating and shaking from the pain. Urologist was called and I told him point blank "Remove this catheter, or I will the pain is unbearable" he said I was having a rare reaction and agreed but warned if I was unable to urinate after the catheter was removed I would need it reinserted and maybe a stay overnight. He then asked are you sure you want it removed as going home you may have to go to A&E at Kings if I could not urinate overnight. I said the pain was to much lets remove it and see. After removing the catheter the pain went from 20 to 6 and reseeded. I was able to urinate by myself straight away emptying my bladder. They then asked me to fill up on water and want at least 50mil of urination. I was able to do 10-15mil per pee but I explained this was the same after my biopsy as then I peed little and often. They offered me a catheter insert or go home but any problems come back to Kings A&E. I refused the reinsert and went home understanding that if I couldn't urinate I would come back to Kings.


Home: Coming home at 9pm I went straight to the toilet. At around midnight I spent a Full bladder, the after effects of the catheter is a painful end neck of the penis. Peeing out a full bladder was painful and urinating is painful but getting better. Seem to go to the toilet every 2 & half hours. This morning feel a bit nauseous and started the 5 day course of Ciprofloxacin antibiotic, 1 tablet twice a day. 


From the operation on the prostate itself I fill no pain. Only when I clinch I can feel a dull ache. Apart from the catheter this time was much better than my Biopsy. 


Now we wait to see the results, follow Up meeting will be in 2 weeks.

A was very concerned about the catheter before the procedure but the 3 days with it in were not a problem, retention after removal was our stress and after doing a self Cath he thinks the best person to put in a catheter is the owner of the penis!

I hope the peeing has continued to improve . Are you off work for a while now?

A also had antibiotics post op but also tamoulison, prednisone and cialis 5mg ( until the side effects kicked in)

We are nearly 7 weeks on from the focal laser ablation so interested to hear your recovery experience.

Hello sorry for the Lack of Updates. 


PC Issues and really not much to add. 


After the op had Blood in Urine and Sperm for a while with no pain. 


Went back to KCH for check ups and Blood checks for PSA which last check dropped to 3. The last check up I walked out on as it was very chaotic. 


Expecting in Nov or Dec another biopsy to see if the Nanoknife was successful or if I need another Nankknife operation.


So next step will be the biopsy and blood test............

i am sure this thread will be of interest to many - interested to see they are gong with another biopsy as next steps, our 6 month step following a focal (although different type) of treatment is PSA test plus MpMRI (16th October for consult).

Sorry to hear about the 'chaotic' experience though!

Hello back with an update. 

Had the biopsy in Feb '18 to see if the Nanoknife was successful. May '18 back to KCH for the results and........
it was unsuccessful

Why? 

The area of the prostate that received the Nanoknife showed tumour regrowth plus a second tumour had grown. Due to having two tumours now I no longer qualify for the Nanoknife Research. Tumours are slow growing and they said even if I did nothing I have 10 years plus as my prostate cancer is  still capitulated. My recent PSA was 5.1.

So now I'm back to the start to have the prostate removed. Problem is I'm a big guy, at the start of this year I was 312lbs but so far I have dropped 40lbs. I still need to drop another 50+ pounds before they will operate. I'm back for a weigh in on November 6th. 

As for this thread, I guess it is the end as I wanted to show my progress on the Nanoknife Research Study for the UK. I was one of only two in the UK on this study. I hope it helps others in the future. The journey and fight continues........

Very disappointing John. Has EBRT been offered as an alternative to surgery and if so have you ruled it out?

I think it would be helpful if you will update your bio with the salient facts so we can follow your progress and where you go from here and how it works for you.

It seems the Germans have been very successful with Nanoknife IRE based on the scientific paper published in April 2019. They studied 471 Nanoknife treatments (treated between 2011 and 2016). After short terms side effects, longer term side effects were very low indeed: incontinance almost zero, erectile problems 3%; and the 5 year recurrence estimate for Gleason 7 patients was 15% (which is comparable to the gold standard radical prostatectomy which achieves 12% recurrence for Gleason 7 patients).

It is likely that results are better in Germany because they have treated many men and in so doing have gained in experience. So while it did not work for Dodgy007 the results on a large scale have to be considered wheen deciding whether to opt for IRE.

Unfortunately, Dodgy does not appear to have told us what further treatment he had and how he is doing.
