Leading Prostat Cancer Hospital

My brother has been diagnosised with an aggressive form of Prostate cancer. Does anyone know the leading hospital/ doctor who can help with a second opinion in the UK?

Hello Kate and welcome to the site.

I can't answer your question but others might be able to with a little more information.

Whereabouts are you based? (What hospital is your brother currently under)

Are you talking NHS or private.

It might also help with general advice if you could give a few of your brother's results.
His PSA level, his Gleason score, the results of biopsies and MRI.

What treatment plan has been suggested or put in place?

Does your brother have metastatic changes to other areas of his body.

Hi Kate,
that would depend really on whether he wanted to see a urologist (surgeon), oncologist or someone who specialises in less common treatments such as cryotherapy, etc. It also depends on what you mean by aggressive.

There is a supposed Top 10 list in England -- these are the urological surgeons that most other urologists said they would choose if they had prostate cancer. But if your brother already has mets (spread) then a surgeon is probably not the second opinion he is looking for.

Thanks for your response. He has a score of 9 but we don't know if it's spread and he is having a PET scan to find out if it's spread. But looking for a specialist in this field to get an opinion of the best treatment. We are an hour from London in Hampshire.

Lyn may know a better list,but if you Google top ten prostate surgeons it will come up with a daily mail top ten list. It is quite old 2010 and is based on a vote from their peers. Things in medical care do move forward quite quickly. I saw one of the guys on the list and was not that impressed.

Not sure if outcomes are included in the way the list was compiled.

I recently decided not to get a second opinion. I'm in Hampshire too , aged 48. After speaking to PCUK nurses and Macmillan with all my history they reckoned I would probably get the same prognosis and treatment plan anyway. I think mostly the NHS are great but I've had a happyish journey with them so far. Best wishes and keep posting

It seems crazy to be seeking a second opinion before you even have a full diagnosis or know what treatment options are going to be offered.

Colwick Chris is not the only one to be less than impressed - another member used one of the supposed 'top 10' and was left with life-changing and permanent side effects and zero follow up support; he described it as a total lack of interest from the surgeon who was perhaps a bit miffed that his success rates had taken a battering.

You can look up the various hospital success rates on Dr Foster's website - urologists have to report their stats annually so you are able to see things like how many men didn't have clear margins, how many needed adjuvant or salvage treatment, how many were stil incontinent 12 months after their op and how many were left impotent.

However, all of that is academic until you know for sure that your brother is suitable for surgery.

How do we go about finding that info ?? I've been on Dr Fosters website. Do you have to register etc to see surgeon's results and outcomes etc. I'm quite interested ( maybe a bit late though )

I just checked - it seems Dr Foster measures different areas each year so the urology report doesn't seem to be available now :-( This year, the focus seems to be on finances and weekend mortality rates, last year it was people being re-admitted and cancelled operations.

Perhaps we should get in touch with them and ask for the urology report to be pulled back out of the archive?

Hi Kate,

I have to agree that without a complete diagnosis it is difficult to see what can be gained at this point by having a second opinion. If the cancer is aggressive it could be considered so because the prostate cores taken at biopsy were assessed to have greatly mutated when considered by an expert looking at them in a lab. There are incidentally at least 27 types of PCa and some of these are more aggressive than others and there is always the potential for further mutation in any of them. What is necessary now is to establish whether the cancer is confined within the Prostate, is in the process of escaping or has spread elsewhere. This is known as staging and scans help establish diagnosis. The patient is then told what treatment options are open to him which also have to take into account a number of other aspects, not least whether there are other medical considerations that would rule out or complicate treatment. Then even if all treatment options are possible, the patient may be more averse to some than others. Ultimately, it is important that the patient gets a highly skilled surgeon with a good track record if surgery is the preferred choice or the most advanced RT that can be had or other treatments. Sometimes where there is a choice of treatment there may be little to choose between effectiveness in dealing with the cancer and then other aspects such as potential side effects can be a deciding factor including what is the next possibility if primary treatment fails.

If in due course you can provide his PSA when he was first diagnosed with PCa, his Gleason score and scans with interpretation, we will be in a better position to know where he is on his cancer journey and hopefully will be able to comment further.

We went for a second opinion to London, a private specialist clinic where a number of well respected Consultants worked from including a Urologist and an Oncologist.

We saw the Oncologist ( who also worked in the NHS) and she confirmed that the NHS treatment that we were being given was exactly what she would do in her NHS hospital and at the private clinic.

the difference was that we had an hour on the phone with her, then an hour meeting with her and then a detailed follow up letter.

It was expensive but very reassuring that we were on the right treatment path for our circumstances.

We had all of the scans and results sent to her in London by the secretary of the Consultant that we were already seeing. Nobody minded getting the second opinion, in fact the original consultant was interested to hear what she said.

I would be happy to recommend this private clinic via the messaging service as we do not mention names on here.
