question about histopathology

Hi guys


I have previously posted about my histopathology showing close margins. I had RP 6 weeks ago.


I now have the proper report copied, the conclusion was as follows:


Robotic radical prostatectomy: Adenocarcinoma Gleason 3+4 overall
(Grade Group 2), volume 1.5ml, right and left side, gland confined, tumour is
seen at a gland incision for 3mm at right posterior (stage pT2+). Apical inked
limit, basal inked limit, seminal vesicles and vasa are free of tumour.


I guess this is what they call positive surgical margin. The question I have is how could the cancer be organ confined if there  is tumour at gland incision.


I am waiting for my first PSA

From my limited knowledge of pathology I would say that’s a good report, and I would be happy with that.

Let’s hope it’s gone forever.

The phrase "gland confined" is the important one here, Thala. That's saying that the tumour was confined to the prostate.

Don’t get too hung up on PSA tests. Looks like you have come out with a good result from your surgery, and even if like two friends who, like me had surgery by some of the top consultants in Britain, they have had slight recurrence and have had salvage/adjuvant RT to kill off any stray cells ‘with curative intent’, it’s not the End of Days!


So they should be OK, just like me. Don’t know the specifics of their biopsies before and after prostatectomy though.
