Hi all I need some advise please I'm due to update my PIP payments when I first applied my dr did the DS1500 FORM can I get another one as I have recently moved and have a new dr not sure if he has all the notes from my previous dr .I have all my notes ,but I would have thought the dr would be able to find them,or is there anyone else I can get it from ie oncologist or Macmillan
