TURP before radiotherapy and HT

Hi all, here for my husband (age59). PSA 23, Gleason 7 (3+4), stage 2.

Offered RP or RT/HT, he would rather go for the latter. He had horrendous side effects to biopsy and ended up with a catheter and 2 infections. Prior to having radiotherapy, he will need to have the TURP procedure, failed the urine flow test.  Has anyone had TURP and how long did you have to wait b4 RT. 

We have been discussing pro and cons of both treatment routes but he really does not want RP. He had a good chat with specialist nurse and really wants HT/RT and not RP. He also been offered opportunity to be on a trial with HT/RT but v unsure. We both just feel lost.

Any advice really welcome.

I also decided RP wasnt for me; been on HT (prostrap injecs after initial bical tablets) for nearly 5 months now and, besides the expected changes 'down below' hot flushes have been the most noticeable side effect to date, a bit annoying sometimes but tolerable. Hoping, once the waterworks are serviced!, to have the RT in 3 to 4 months time. Good luck with the treatment.

It isn't that men have to wait for RT. He will have at least 3 months of hormones to weaken the cancer before they start planning the RT and some oncos prefer 6 months. Research shows that the longer he has HT before starting RT, the more effective it is likely to be.

So havinga TURP shouldn't be a problem.

I had TURP in May 16 with HT (Zoladex) starting Nov 15. RT, which was 37 sessions, began 1 Nov 16 although it could have been started earlier but put back a bit as I got married 26 Oct. Can't remember when RT was originally scheduled but there was no panic as HT was doing its stuff. I was also part of a trial, stampede arm j having abiraterone, enzalutimide and prednisolone for 2 years.

Looking back, the TURP was fine, didn't take too long to get over it and it made a difference. As I hadn't had any 'proper' hospital procedure before, luckily, it was a step into the unknown etc but it was fine.

Dawn, I went down the HT/RT route. Happy to answer any questions you may have about it. As Lyn says, your husband will be on HT for a while before the RT (I was on it for 6 months prior to RT), so no rush.

I was diagnosed in January 2013 and was started on HT (Prostap).  I had a catheter in place before the TURP because of retention problem.  I had the procedure in June 2013 followed by RT in September 2013.  I had the TURP on the Friday and came out of hospital the following Monday.

Glad you've made a decision Dawn. Things are always better with a definite plan! I've just had my test results back two years after my RT and it's all looking good. Hope your husband's treatment goes equally well.

Thanks everyone for all your advice and support, we have finally made a decision over treatment route and John will soon start HT/RT, definitely needs TURP first but after speaking to specialist nurse, I think this is best for John given his previous issues and side effect of biopsy. Nurse indicated that if he had RP, he could have some longer lasting effects of incontinence. He feels so much better that he has made the decision. He is going standard route for RT, 20 fractions over 4 weeks, offered opportunity to take part in a trial which involves RT over just 1 week but risks and side effects a bit unknown and whilst research is so very important, I think given the issues he has with urine retention, he feels more comfortable having RT over a longer period. We live near the hospital and he is unable to work so feels this is the best route.  Will keep you updated on progress and any side effects along the way.
