Heart attack, low dose cialis

Hello all

Would be grateful for any advice.

Had radical prostatectomy for localised prostate cancer 3 years ago and as yet no recurrence, thankfully.

Just when I had accepted my Prostate diagnosis, unfortunately had a heart attack 4 weeks ago and had a stent. Not sure when to restart low dose Cialis 5mg for continued penile rehabilitation.  Conflicting advice from GP and others for this aspect of treatment. All agree that 'high dose' should not be had for 3 months or so, but I feel should be safe for low dose Cialis. ED just starting to improve and dont want to lose the benefit of low dose Cialis.

Anyone else have had advice/experience in this regard?

Regular low dose gives the same level in the blood as event dose - it's just there 24x7 not just when you need it.

TBH if you are getting a response it may be time to try without it. A pump will probably deliver more benefit in terms of rehabilitation.

Now could be time for a simple cock ring (Durex play type NOT a pump constriction ring) too, I found it works better, is more reliable and doesn't give you a heart attack!

The 5mg daily low dose Tadalafil gives you equivalent to an 8.5mg once-off dose, because of the compounding and half-life from previous days too. That's quite some way off the 20mg event dose. Although thought to be good for penile rehab for blood flow, the daily low dose is insufficient for those needing substantial PDE5i help to get erections, but many people needing PDE5i help don't actually need the full 20mg, and the 5mg daily low dose is enough for them for regular erections.

If you're on 5mg daily low dose and you want the effect of an event dose, you need to take 3 of the 5mg tablets in one day, and can then skip them the following 2 days to get back in sync with your calendar pack. That gives you the equivalent of 18.5mg once-off that day, very close to the 20mg event dose. The day afterwards you are still at same dosing level as if you'd just had 5mg daily, and the following 2 days (which includes the day you restart taking 5mg daily) you are below the 5mg daily dosing level, but still have some continuous dosing.

Both the 5mg and 20mg tablets are available half size, 2.5mg and 10mg. These are for people who find they get unacceptable side effects, but the half dose is OK regarding both intended effect and side effect. The half dose tablets aren't usually cheaper though.

I don't get hard enough for sex with 5 mg Cialis, need higher event dose.

Is it safe to continue 5mg post heart attack?

My doc says 10mg is an event dose, only use 20 if 10 isn't enough.

You need to ask your cardiologist or GP what is safe.

Have you considered alternatives to drug treatment? I find the use of vacuum pump gets me nice and hard without any risky side effects.

I’m sorry you’ve had to deal with all of that back-to-bac, sounds really tough. I’m a guy in a similar boat in that I’m also on Cialis 5 mg daily right now, from this online-pharmacy: [Supplier link removed by moderators], and so far it’s been going well for me. From what I’ve been told and experienced, low-dose daily tadalafil is generally considered much gentler on the cardiovascular system than on-demand/high doses, provided you’re stable after the heart attack and not taking nitrates (that’s the big no-go). Many doctors are cautious by default, which is why you’re hearing mixed advice.

That said, after a recent MI and stent, the final word really should come from your cardiologist, not just the GP, specifically asking about daily 5 mg for rehab, not “ED meds” in general. In stable patients, some cardiologists are comfortable restarting low-dose tadalafil relatively early (often a few weeks) if blood pressure is controlled and there are no symptoms with light exertion, but they usually want a quick check-in first.

Explain clearly that it’s penile rehabilitation, not sexual performance, confirm you’re not on nitrates, and ask for explicit clearance for 5 mg daily. That way you protect both your heart and the progress you’re finally starting to see.

Patients usually go get these drug stimulants (and pumps) prescribed by their GP or Oncologist.

You don't show any detail under your Profile. Do you have any expertise on this subject?
