Radical prostatectomy op

Hi, I underwent a radical prostatectomy in August 2021. The surgery went well, no incontenance and was told last week all margins clear. Since the catheter removal I have been getting really painful, the only way I can describe them are spasms when I go for a pee. The pains starts after I pee and they seem to be in the rectum area and travel to the testicles. They last for about a minute and start slowly getting more painful then subside. Only seems to happen at night as no problem during the day. I can’t take ibruprofen as I only have 1 kidney. Anyone have the same symptoms?

Could be an infection , best to get a urine sample to the GP. You probably still have bits of scabs and debris coming away from the surgery.  Even a small particle passing through the urethera will transmit pain to the other parts of the water works system. 

Pain when starting and or stopping urination is often mentioned, that doesn't mean it is normal. If in doubt always get medical advice,  our urology department has a team of nurses who will answer questions like yours.

Yes I had contacted the specialist nurses in Cambridge and was also told to take a sample. I did and this came back clear. I had also spoke to the surgeon last week when he called and I also mentioned it to him. He did say it was major surgery and early days and this should ease over time.

Most likely explanation is that it is the nerve endings jangling as they heal. Your flesh has been cut and burnt; as it heals, it can send all sorts of weird and confused messages to the brain. You have also had a vasectomy so bruising and pain around the vas deferens and scrotum is not really surprising.

Hi Lyn

Also after speaking with the surgeon last week he did say that 15 lymph nodes were also removed. He did say it will take time to settle. Let’s hope it does sooner rather than later.

Hi all

I am now 12 months post op. Have had 4 psa tests the most recent last Monday. The first one 4 weeks post op was undetectable 2nd 0.01 3rd 0.02 and the most recent 0.04. GP says still very low don't worry. I have been reading that dehydration can have an effect. So I will not have any alcohol the week before my next one which is in 4 months time. I will also drink plenty water and hopefully the readings will stay very low. Has anybody else had similar.

Yes don't take advice from GPs on PSA post op they are worse than useless.

Freddy, 

I am nearly 10 months post op and have had three tests. First was less than 0.06, second 0.06 and third,  just last week,  0.08. All quite low but increasing nonetheless. I have my scheduled consultation with surgeon on Wednesday and expect him to take no action,  pending next test.

As expected, Freddy, no action now, next test scheduled for November. However, if the rising trend continues, he will refer me to oncologist.
