Long term radiotherapy side effects

Hello, I received radiotherapy back in March and recovered well with little side effects until six months later when I started to have pain while urinating as well as severe pain in the rectal area with soft stool. Wondering about other people’s experience and if anyone has gone through the same things. Thank you.

These are both possible late-onset side effects (meaning not starting until at least 3 months after treatment, but can be years later), known respectively as radiation cystitis and radiation proctitis.

For the radiation cystitis, I would start by getting a urine dip test done, looking particularly for infection markers (although these aren't reliable in dip tests), and also for microscopic hematuria (traces of blood too small to see). In case of infection, you may need some antibiotics, and drinking cranberry juice might also help. Any case of hematuria needs referring back to urology on a 2 week wait, just to confirm is it radiation cystitis and not something more serious.

For radiation proctitis, you would be referred to colorectal, again for them to confirm this is the cause, and not something more serious. If you pass blood, this would also be on a 2 week referral.

Just to add a bit more, late onset side effects can sometimes go away by themselves after a while.

I got rectal bleeding 5 months after radiotherapy, and it's been going on for 18 months. In my case it's not painful and isn't a Quality of Life issue, but I did get it checked out just recently to make sure it was radiation proctitis, and not for example bowel cancer. I have been given a steroid foam to spray up where the sun don't shine, but before I got to start using it, the rectal bleeding has started reducing and I'm going to see if it stops by itself, which does sometimes happen.

I had bouts of radiation proctitis on and off for about two years after RT. In each case it would last about a week and then fade away, and the interval between them gradually increased until it stopped altogether.

Definitely worth getting advice on.

Thank you, Chris. Did you receive any treatment for the radiation proctitis?

I’ve seen my consultant and so far he hasn’t offered a strategy to lessen the discomfort. He has done a urinary check for bladder/kidney infection and has referred me to get a colonoscopy which I understand is the usual practice.

Aside from the urinary pain and soreness in the prostate area, I’m now experiencing a constant need to urinate both night and day, and I would appreciate any further advice/info you can give.

I delevoped stage 4 radiation cystitis, a few months after salvage RT. By the time my pelvic radiation damage was diagnosed i was beyond help, I now have a catheter for life and a bladder capacity of twenty mls. My symptoms were passing blood ,blood clots and tissue. Urology and radiotherapy blamed each other. 

Don't get fobbed off and if you can get a referral to someone who understands radiation damage. Severe damage only happens in around 5 percent of some RT treatments.

It was external beam radiotherapy. Not sure about greys, but it was 5 sessions administered every second day.

colwickchrisI'm so sorry to hear of your predicament and thank you for your advice regarding finding someone who understands radiation damage. I shall look for a specialist (if they exist).

The pelvic radiation disease association were very helpful to me and recommended a specialist in a hospital about an hour away from me.  Check out their website and give them a call

Sorry for the delay in replying - I've been away for the last week. No, I didn't get any treatment for the proctitis; fortunately in my case it just gradually lessened and had entirely stopped about two years after RT.

Been really good 8 months after 37 lots of radiotherapy bit off a setback last week had a small anal abcess got antibiotics cream off doctor's hopefully clear up soon surprised after 8 months but no other problems really hoping it clears up soon gaz 👍

Nearly a year after 81GY delivered as 45 fractions over 66 days, I've had no side effects. Apart from more frequent urination and minor irritation during RT there has been nothing to indicate any beams had damaged any targets other than those that they should have.
