Dry on day 1 after RARP?? Should we be worried?

Hi all


Thought I'd posted this here a few days ago but can't see it on the message board. Must be losing it! 🤪


My OH had RARP 11 days ago. Catheter came out 4 days ago. We were told he would be completely incontinent at first and we were expecting Niagara Falls. However, after catheter removal he has been dry and not leaked once.


At first we were worried that he might have retention and we went back to the hospital for a bladder scan. But he is managing to wee. It's just that the flow is very slow and he almost feels like he has to force out the urine when he's going, which can be sometimes painful. He also doesn't feel able to urinate standing up as the flow is not strong enough. Anyone know, is this to be expected? It's something that wasn't mentioned to us. All we were told was that he would be incontinent immediately after the op. We're now faced with a situation totally different to what we expected and not sure what to do. 


We're either extremely lucky with the lack of incontinence or something is wrong. We don't feel particularly supported by our oncological nurse (she always makes us feel like a nuisance for asking questions) so just wondered whether anyone on here had any ideas or advice?

Some guys are dry from day one, I was 99 percent dry four days after catheter removal. His pee pipe has been cut right through and stitched back together, it needs time to heal. 


Keep an eye on the flow rate, it easy to check you flow rate.


If in doubt get in touch with your medical team.

My surgeon assured me before my op that I would not be long-term incontinent. And he was right. Eight weeks post surgery I was dry. Every one is different.

I was pretty much dry from my TWOC, my flow wasn't that good either until around 4-5 weeks when the last debris cleared from the new join in my urethra. After 5 weeks, I was peeing like a horse. 


Perhaps this could be the case with your OH? Could there be some scabbing in the urethra that will clear in time, or maybe a urinary infection? 


If you can't speak to a urologist, could you try one of the prostate cancer nurses?

Hi, I had a urine infection post op, and I think many of us on here have too, so I don't think it's uncommon after having the catheter in for ten days. 


Hopefully the antibiotics kick in and clear it up soon.

I had one too, although it did not affect flow. I was also pretty dry from the off and I was 68 at the time of the op.

Robot, my symptoms were, a water infection, very smelly urine and a slow flow. I think somebody already mentioned scabs and stitches dissolving, they can have a similar effect. My stricture was not discovered until about 3 months after surgery. I remember passing blood at work after surgery, so that must have been at least 5 weeks after surgery. Blood in urine does often look alarming.

Robot just another thought I now have a permanent catheter, I sometimes get clots , my urine ranges from clear as tap water to red wine. The catheter can cause trauma in the bladder. Sometimes my urine can be red wine colour and two hours later quite clear. It doesn't take much over doing things to cause blood in my bag.  Accidentally catching the catheter will also cause blood in my urine.


The main concern for you would be a problem with the new joint, I was told urine leaking through the joint into the abdominal cavity would make me very ill. On one occasion work on my stricture, (which is at my joint) did result in bright red blood and clots hanging out of the penis at the side of the catheter. I did go into hospital for a few days but only had a couple of bladder flushes.


Hopefully it will all settle down soon, but if in doubt get onto the hospital.

He's now been diagnosed with a urine infection and has been prescribed antibiotics. Bit slow in diagnosing imo as he had the test a week ago. Anyone else had a urine infection post RARP?

17 days post RARP and 10 days post TWOC and we’re back at the hospital on Christmas Day. OH unable to wee. He can only pass a minute dribble and it’s bright red with the occasional clot. Was diagnosed with urine infection a few days ago. Don’t know if that’s anything to do with it?!?! Had to leave our young son at home with my mum playing with his presents. It’s heartbreaking. Christmas dinner is a right off! Hospital is one hour 20 mins from where we live. This all feels like a total nightmare. I thought things were meant to be improving!

Robot, sorry to hear that you are back in hospital, a few of us on here have had issues with surgical clips migrating into the bladder or urethra. It is supposed to be a 1 in 500 event. The clips are deliberately left in and should just settle in corner of the abdominal/pelvic area, it isn't negligence.

Thanks Chris. Yes I’m going to ask about that possibility. He was sent home with a catheter in yesterday for another 7 days. Bit of a pain really. But our main concern is the blood. His urine is still really really red. Getting quite concerned. Might have to call the hospital back today. He’s drinking loads of water to try and flush it through but it’s not really making a difference or running any clearer. It’s now been 18 days since the op. The doc said yesterday that it wasn’t a stricture because the catheter went in really easily. Can I ask Chris, when you had clip migration, what were your symptoms?

After undergoing RALP on 21/12 and due to have my catheter removed on 30/12 I am keeping an eye on your thread to see how things progress. My urine after 5 days is a little bloody but certainly not the colour that you have said your OH's is. As a matter of interest, was your OH given antibiotics to take for a couple of days before the catheter was removed?

no. But he found out he had a urine infection a week following the urine test!  Maybe it’s related. He’s been on antibiotics for 4 days though now (course of 5 days)

Quick update... OH back at hospital today (so we've now got the full collection of Christmas Day and New Year's Day!) Same thing happening. After having catheter put back in on Xmas day, he had it removed on Thursday (30). he was doing fine. Urine was clear, flow was okish for just over 3 weeks post op. But then last night (NYE) around 11pm he started seeing blood in his urine and eventually it stopped although to just a bloody drip. Had him back to the hospital (an hour and a half away from us) today. They've put another catheter back in for another two weeks! I know it's still early days but it all feels like a total nightmare and not what either of expected at all. Really hoping it's just the healing process and blood clots etc and not something more sinister that is going to have a long term impact. We asked about possible migration of surgical clips but the urologist told us it would be too soon post op for anything like that to be the cause. Really really hoping it's something and nothing.
