I hope to be moving home soon.  I’m moving within the same county but the town will be covered by a different NHS Trust to the one where I’m living now.

I’m really impressed with the team that have cared for me since 2012 and have total confidence in them.  

I would like to stay with them if I can - is this possible?

Would there be any problems if they issue blood test forms etc and I have the samples taken where I live?

Future treatment if required?

Your advice would be appreciated and if there is anything I’ve missed please let me know.

I don't think they will make you change to a new urologist Arthur - the blood test issue will need to be discussed with your new GP but lots of hospitals stop issuing the blood test forms after a couple of years and hand all that over to the GPs anyway.

Do you have an Oncology specialist nurse. ?? I just message mine and she sends them through the post !! Wherever you have a test done it will be reported back to your team ( the issuer ). You need to inform Oncology of your new GP name and address , and then he/she will be copied in

My GP and hospital can't see each others blood test results, even though they both go to the same hospital lab for analysis. On one occasion, I had a form from both of them with some common and some different things. All analysed from a single blood phile, but they could each only see the results for the tests they each requested.

I always transfer my blood test results into an Excel spreadsheet and take it with, so they can all see everything. I can see a few copies of this spreadsheet over time have gone into my hospital notes. My consultant has got used to this and always asks me for blood test results, knowing they're easier to see and more complete than on his computer.

Hi,

Where I live hospitals in different places specialise for several hospitals.   My prostate surgery and aftercare is done at a hospital 15 miles away and my GP gets paper letters about my condition.   Whereas I know he's linked electronically to the hospital that is just over a mile from our house.   The nurse sends blood test forms by mail to me.   

Also you can ask to be treated in other hospitals so it doesn't sound like it should be too difficult in normal circumstances.

Thanks for your replies.

I’ve now moved, registered with a new GP and had my medication review today.

The GP said I couldn’t use the blood test form here issued by the trust that I’m being treated by, he said if he was to issue a blood test form the results wouldn’t be seen by my oncology team. I got the feeling he didn’t want to do it.

I don’t know but I would have thought that my record would be accessible.

He felt the best option for me was to go back to the hospital who issued the forms for the blood tests which is possible but not a straightforward journey from here but if I have to that I’ll do it as the team treating me have been first class and know my history.

Has anyone else been in this situation?  Any advice appreciated.
