Morning,

Hopefully i could get some help/advise for all out there going through similar diagnosis.

I have a appointment to discuss my RT options on the 20th May.

I had my PSA level checked couple of days ago and unfortunately it has risen from 2.6 to 3.5 ( originally it was 88 then 9.2 after a month then down to 2.6 within 5month )

Has anybody else had similar changes and is this common ??

I have several lymph nodes with cancer spread out within the pelvic area and we are hoping that the specialist says that RT will catch everything when treatment starts ??

All input would be much appreciated.

Hopefully, RT will zap all the cancer. Due to the way RT works on cancer cells, it is likely to be many months before your PSA is pulled down to your nadir, so a wait and see situation. Meanwhile following RT you may experience small rises and falls of PSA with successive PSA tests. It is only if these continue to trend upwards overall that there is generally cause for further consideration.

Given your rising PSA, I think you need to start RT ASAP, but you might need to add another hormone therapy treatment. I would ask if you can add Enzalutamide, although this has only been done in a trial at your stage in the UK so it's not currently NHS standard care at your stage, which means they might not be able to fund it. Another reason not to is it might prevent you from having it in the future if it was needed then. It would be a good discussion to have with your oncologist.

I have an appointment on Friday with the RT team to discuss as my PSA over the last week or so has risen from 3.5 to 4.9

My testosterone level was checked and its a 1 which they say is good and kidney test is also fine ??

your assistance is great and any for info would also be great

Your Testosterone at 1 is below castrate level (usually held to be around 1.2). However, some people do go significantly lower than that, and it might be worth discussing if trying one of the other injections is worth a go, given your high Gleason score. I've come across men who transferred from Prostap to Zoladex to get a lower Testosterone level, but also one guy who went the other way to achieve lower Testosterone.
