Swollen thighs - a very bad sign?

My husband was diagnosed six years ago and the cancer has now spread to a small area on his femur.  The medication he has been taking for 8 months is enzalutamide, which, so far, is keeping his PSA level very low.  However, he now has swollen thighs and some discomfort around the area where the bone cancer is situated.  We'll be seeing the oncologist again next week, but this has made him very depressed and he sees it as the 'next stage' of a downhill path.

I'm presuming the swelling is something to do with the lymph nodes, but would appreciate anybody else's experience of this.  Is it really such a bad sign, or just something which happens to some?  I presume that the oncologist will be able to help with this.

What is his PSA doing? If it is going up the cancer is probably active, but if it is not going up then the swelling could be caused by something completely unrelated.

Thanks for your reply, Dave. His last blood test was two months ago and at that time his PSA was well below 1 - as it has been since he started the enzalutimide last October. He will be having another blood test before we see the oncologist next week, so, fingers crossed his PSA is still very low. His hands have been swollen ever since he's been on hormone therapy and the specialist nurse told him a couple of years ago that it was likely to be a side effect of the medication. He was only on decapeptyl injections at that point and he is still having those, as well as taking the enzalutimide. So, I'm hoping that side effects are the reason for the further swelling.
