Worried about time for template biopsy

Hi since February  2019 it’s been a real rollercoaster trying to be diagnosed, around that time I had an infection so was on antibiotics and doctor did a psa test that came back high I think 9.2 so with a family history my uncle dying from prostate cancer the doctor recommended a mri scan.

June 2019 mri showed T2 possible local prostate cancer so the doctor referred me to the local hospital urology department, went to see the consultant beginning of June did a DRE one side firm so said I need a biopsy and he was certain it will come back positive for cancer.

August 18th 2019 Trus biopsy 18 needles the result for this come back around end of August negative so I was hopeful it might be just an infection or something. Appointment September specialist nurse said at this meeting that they would keep an eye on me and to book another psa test around end of 2019 so I think I got tested around the 13th of December.

Next appointment at hospital February 19th 2020 seen the same consultant he said my psa had gone up to 10.2 so recommended a template biopsy which was a bit of a blow when I was hopeful I might be clear but said waiting times could be 6 months. Well by march 2020 the world changed and covid and lockdowns took over and the rest of that year was lost.

Fed 6th 2021 at last they got me in for template biopsy turned up 7.30am in morning and to cut a long story short they would not do it I think I had some diarrhoea a week or so before so said I would have to go and get some stool samples at doctors done and they would try and get back in about 14 days later, they was worried about some kind of infection but I can’t remember what it was now. 14 days became months and I did get a call around June 2021 saying if I agreed instead of a template biopsy that I could have another mri scan and up to date psa test so went for that option.

June new mri scan done I think it must have been two weeks later nurse called me saying they had seen changes on my mri scan and I need a template biopsy.  I had the call for appointment not to long after in mid August 2021 while I was away in Newquay on Holliday. I came back from Holliday all ready for this biopsy and luck really was not on my side and end up with Covid-19 that I caught on Holliday so could not make appointment. 

From that missed appointment nothing happened for the rest of 2021 and out the blue January this year I had an appointment come through for a body CT scan with contrast I went for this scan and had a letter for a phone consultation with consultant towards end of January. From this phone call he said that the area in prostate had got bigger and I was going to get a appointment for a trus biopsy again because it would be faster.  14th of feb this year had my second biopsy 4 needles in the target area and random needles both sides of prostate again so around 16 in all.  Did not have another phone appointment booked until 13th of April this year but between the biopsy and phone call they fitted me in for a choline pet ct scan.

From that phone consultation the consultant said that the biopsy had come back negative again and the pet scan had shown one area in prostate and one suspect lymph node so the recommendation is I need to have a template biopsy that I might have a bit of a wait for. It’s now been around 5 weeks since that call and they finally sent me a copy of the letter they sent to my doctor saying  about the changed in mri from T2 to T3a 1n and the rad 5 change. I really don’t know what to think now so much time has gone on when I might have had a chance of a cure now it’s  looking more unlikely all the time if only one of the 2 trus biopsies would have shown the cancer I could have been treated, the consultant said it still might be nothing I find that very unlikely with the rad 5 T3a 1n findings on mri, I’m sorry for such a long message but I find this all so worrying and frustrating at the same time.

Hi Gary,

Sometimes circumstances work against you. I experienced this last year causing much delay before getting my second HIFU treatment. Goodness knows how much longer I might have had to wait if I had not chased various things up several times. In your situation I would write to your hospital pointing out the long wait you have had in getting a firmed up diagnosis and the concern this has caused you. Hopefully, you will then get the template biopsy soon. Scans do not always show cancer which the template sometimes finds.

The important thing is that any cancer you have is contained within the capsule and if this is the case you have a good chance of treatment eradicating it or at least setting it back for many years. So don't despair.

Well I finally got my template biopsy the 13th September what an experience that was had it under GA that went well but ended the day with water retention so needed a catheter for 15 days I found that the hardest to deal with, plus the fact while I was under they ruptured my hamstring and could not walk for a month.

On the 13th September they said I would have the results in 2 to 3 weeks but it’s now 9 weeks they have  still not let  me know but have  booked me an appointment for this Friday to see the nurse 9 and half weeks after biopsy, I hope it’s 3rd time lucky that this is the  last biopsy I need to have,  first was an 18 samples truss then a targeted biopsy in February 15 samples and this template was 50 samples. The fact they keep doing these biopsies must mean I’ve got something just keep missing it. Just a bit worried if they found something in the first 2 to 3 weeks why wait almost 10 weeks to start treatment.

Hi Gary, yes completely understand your frustration. My first diagnosis was in May 2019 PSA was 6.8. Had. Trans perineal templated biopsy (42)which was T2 and one core with 5% cancer. My MRI did show an abnormality. Have been on Active Surveillance since then with irregular tests. With your situation you should phone the Urology Nurse Specialist at your hospital and get them to help you. Thankfully my Urology Nurse Specialist is very good especially as I was missed and overlooked but once I called it went into overdrive. Had to have another trans perineal templated biopsy on 1st November 2022 (71) because my psa had gone up which it has been since started but only a little bit at a time. There was extra cancer found but it is still T2 with Gleason score of 6. Still on Active Surveillance so will expect another biopsy down the line. The best thing you can do is to keep phoning your Urology Nurse Specialist to find out the exact details of your biopsy. There are treatments even if you are on Active Surveillance. Plenty of reading material on this website. Learning more every day about prostate cancer and how complex it can be. It’s never a one size fits all. We are all different. Keep your pecker up mate.
