Hi,I am 74 and was diagnosed with prostate cancer in June 22 due to having a blood test with a PSA of 7.02.Had a biopsy following an MRI.The result was localised PC with a Gleason of 3-4,with performance score of zero.

I was put on Zoladex 10.8mg. on 21st July.The only side effects I have had are Hot Flushes.I have had dry orgasms and have recently had my second Zoladex implant on Oct 13 th.

I had initially chosen to have Radio therapy with the Hormone treatment.I had a Face to Face consultation with my Onco on 7th Sept,she said I could almost do Active surveilance instead.My father died of PC at 64 my brother had it at 57 but was "cured" by having a prostatectomy.Because of this she said I might as well go ahead withRT and at the time I agreed.

Now ,my problem is this, on the 23rd Sept I chose to have a psa test and the result when I was still on the first Hormone treatment was 0.5 only!Also I can get full erection and reach orgasm(dry)

I find this strange because the implant is working(ie hot flushes, dry O etc)but every thing else is working!surely this is not supposed to happen!

Can anyone throw some light on what's going on here?Thanks.

Good for you! I'm also on HT for six weeks and have only noticed hot flushes.  No other side effects so far or loss of libido.  I suppose the response varies between individuals and some side effects may take longer to manifest themselves.

My Gleason was 4+5, T3, PSA 25, Age 53. Doing nothing was not an option for me. I had RT and HT. With your numbers Active Surveillance might be possible, I think within 10 years you would need treatment. You are coping well with HT, the RT doesn't usually have troublesome side effects.

If you were at the start of your journey and very worried about treatment, I would have suggested you take up the offer of AS and see how quickly the PSA is rising in six months. Now you are on the treatment band wagon, I think you should go with the plan.

I’ve been on HT just coming up to 2 years. For the first year I was still fully active sexually but I think time takes it’s toll. In the second year I felt my libido dropping more and more , but tbh I can still get a good erection and orgasm ( although orgasm has become more elusive ). It helps that my wife is full menopause and we really enjoy lots of affection and cuddles

HT can take a long time to take full effect on your Sex life, perhaps you were oversexed a bit before all this?
I would get under way with the RT if I were you. Tadalafil can offset the Sex problems, that HT brings you - it's really a weak viagra ! It can give you tummy upset though.
Just remember, there is no Sex, after your dead.....😅

There's nothing abnormal about what you're experiencing. Now you're on HT your PSA will drop rapidly because the cancer is being starved of testosterone. The response to HT in terms of libido varies a lot between individuals. In my case I never lost the ability to achieve erections, just the desire to do so.

It's obviously your decision to make, but in your situation I'd go ahead with the RT personally. You could have another 10-15 years ahead of you, and were you to go with AS, radical treatment will almost certainly be required at some point. Better to get it over and done with!
