39 years old, high PSA, seen specialist twice but asked to wait until 40. Should I decide on surgery

Hi, I’m Asian. I had a high PSA reading as part of a routine health check when I was 31 in 2015 slightly above 4. I saw a specialist who told me I’m too young to be considered a candidate, he did a DRE and MRI with no abnormal findings, gave me antibiotics but the PSA didn’t go down, but he suggested wait and see. I saw another specialist in 2019 and did another PSA which was about 5 who told me the same thing and told me to come back when I’m 40.

I have no family history, I don’t smoke and am only a moderate meat eater. I recently did another PSA in 2022 December as part of routine health check and had 4.9, and my ESR rate is 27 but I’ve not seen a specialist yet. I’ve researched and thought about this a lot and decided that I may want to do a biopsy and perhaps prostatectomy.

I’d like to seek some advice on pros and cons of biopsy and prostatectomy in my case. I’m also wondering if I can insist on having a prostatectomy done even if assuming biopsy results is negative. Also, can I ask for a biopsy immediately if the doctor ask me to wait and see again?

Hi NT,

You don't show your Country and if UK have been seen within the NHS.

It is for your Consultant rather than any of us to advise you about your treatment but I am surprised that you seem to want to bring forward a Prostatectomy before this was recommended and thereby risk what could be adverse life changing adverse side effects. I think you would have to present a very strong case to have this surgery within the NHS before it was considered necessary.
