I was 63 when I opted for surgery (my details can all be viewed on my profile) and my numbers were quite similar to your father's. I opted for surgery partly because I was fit enough to undergo the procedure, partly because the treatment does not go on for months and months (Although the side effects can and often do) but mostly because I wanted the  cancer that was contained within my prostate out. There are different side effects when undergoing surgery and/or radiation, but the outcome is broadly similar.

It really is down to personal choice and what your father feels is the best treatment for him based on the how he will cope with the different side effects.

For treating early cancer most men will be offered one of the types of Surgery or Radiation. There are other treatments that largely come under Focal Treatment. However the criteria for having these is more restrictive and most hospitals don't mention them as they are unable to offer or do not favour them, so it means seeking out a Focal Treatment specialist if interested. There are pros and cons to all types of treatment. A good place to start is to read the 'Tool Kit' as here https://shop.prostatecanceruk.org//our-publications/all-publications/tool-kit?limit=100

Well the fact the PSA is down a little is good. with g3+4 and quite a lot of cancer this would probably lead to his death in about five years, and the last year or two would be quite unpleasant so yes treatment is a good idea. Surgery and radiotherapy are equally effective so yes look at which has the better side effects for your father.

What does your father's consultant say? Mine said that normally he felt there was little to choose between RT and surgery, but in my case, because of the large size of my prostate, he recommended surgery. I would suggest doing plenty of background reading so you are informed about side effects etc, but I would be heavily influenced by the advice of the consultant.
