2 years post RP - ED wise - might this be as good as its going to get?

2 years post open RP and PSA <0.01

Waterworks very good (have been since the day the catheter came out)

Erections unfortunately not so.

53 years old generally in good health. Zero problems with ED before the surgery. 

 

Just got back from a really down beat and upsetting review with a urology consultant (not the one who did the surgery) where she told me that erection wise this is probably as good as it’s going to get. Saying that if it was going to 'get better' it would have done so by now and as it’s almost 2 years since the surgery it’s unlikely to improve further.

 

I use the vacuum device 2/3 times a week and have tried a daily 5mg dose of tadalafil  – these tablets make a decent improvement and when coupled with the infamous durex ring I can muster a rather impressive semi which is almost capable of intercourse but not quite - but the tablets make my legs ache really badly so haven’t used them for about 6 months now. Got a request in with the GP to try some smaller dose 2.5mg tablets to see if this helps with the leg pain.

Tried a full single dose of tadalafil - same effect as the daily dose but much worse leg pain. A full dose of sidenafil made my head feel like it was going to explode so haven’t bothered again and I’ve tried Vardenafil a couple of times but it didn’t work as well as the tadalafil. 

Really p$$$$d off with the whole thing now and it’s really starting to upset me much more than I expected it would and want it to. 

I think I’m doing everything I can to help it recover further but do I need to maybe accept the fact that this might be as good as its going to get ?

Philip, I have toned down what I originally wrote regarding your consultants comments.

There are many on here who will testify that improvements to their erections have gone well beyond the two year mark. I am 9 years post surgery and I am still improving,I don't expect I will make a full recovery but then again I was supposedly non nerve sparring.

Never give up and have fun looking for a solution. I have often posted that you don't need an erection for vaginal intercourse, look up flaccid insertion or flaccid intercourse. Keep going. Get the injections if you can, although it was dismissed on here I found my long time penile health improved after I started using the injections.

Try injections for a while, seemed to "wake up" some lost potential for me. Why didn't the consultant suggest that? Mine did and he was right.

If you are getting a response to the pills injections should give you a stiffy like the old days! The problem will be walking up and down stairs for an hour to get rid of it afterwards!

I am 8 years this year, 5 years since last injection, and I think it's as good as it's going to get. But with a durex ring it's good enough for "facing each other" positions.
Still not mastered doggy but probably because I am having to much fun to try!

I was "partial nerve sparing" in case you were wondering.

Hi Philip I’m 55 and now incurable. I did everything possible to regain EF after surgery and had great success with natural erections at the 2 1/2 to 3 year mark. I did take daily 5mg Tadalafil at nights though and used pump every time I had a shave. At your stage I experimented with injections and didn’t mind doing them. Caverject and Viridal never really worked for me and left me with a penis that felt like it had been slammed in a lorry door. Invicorp25 though was a GREAT success. No pain , full penetration and easy to do. But I got them privately at £100 for four. Not sure if NHS prescribe. Good luck , don’t give in and may all your courgettes be marrows 👍

Thanks that has lifted my spirits :-) Injection wise - there looks to be a couple of them , is one 'brand' better than the other, if i get a choice is one more preferable to the other?

It is a common medical view that wherever you are at 2 years post-op is where you are going to stay*. But take heart, Philip! John made the most significant progress in months 24 - 36.

Similar to you in terms of what was tried except the side effects of the tablets are not unbearable - Levitra works better than Viagra in our house. Things were better once we realised that a) you are supposed to take them on an empty stomach and b) they don't work immediately ... if J wants an erection at bedtime, he needs to take the tablet in late afternoon / teatime, for example.

We have swapped from Caverject to Invicorp, mainly because of supply issues with Caverject a couple of years ago.

*I think it probably is true that a man who has not been able to get any kind of swelling (through vacuum, tablets, injections or whatever) by the 2 year mark is unlikely to ever recover natural erections, due to priapism as much as anything else, but if there is some response / swelling, there is always hope of it improving further.

Philip, I originally used muse pellets,which come in a applicator that you put down the eye of the penis, it isn't as bad as it sounds, they were a bit hit and miss. I changed to caverject injections and only used that one brand, so cannot comment on what may be best. Have fun.
