Chemotherapy vrs Radiotherapy

Healthy 54 years; Gleason 9; Staging T3b N1 M1.Metastatic spread to pelvis and evidence of skeletal metastasis (Right Rib) makes treatment option not straightforward. PMSA pet scan showed tracer focal on rib therefore oncologist dilemma whether high or low burden.  Oncologist today has proposed 4 options for consideration:

RT / HT

3rd Gen ARPI ( Enzalutamide or Apalutamide)

Docetaxel Chemotherapy

Docetaxel Chemotherapy + Darolutamide (3rd Gen ARPI)

Any advice on pros / concerns short and long term or expereince of treatments. Thanks

If you are being offered daralutimide and chemo as part of a trial, I would give that serious consideration. Men on trials are monitored more closely and, at the moment triplet therapy is only NICE approved for men with no mets but results are very good so hopefully will be just as effective for men with mets
