Hi all 1 year down the line and still clear on my 6 monthly blood test.  Compared to some I know I am very lucky since prostate surgery and at 53 I hope it continues. My question today is I am struggling a little with my sex life erections are not great even with taking 4 Tadalafil tablets before sex. I am waiting for a appointment at urology to see about injections into my penis. I have a pump but find the rings hurt my wife a little. I can get a semi erection and some days are better than others. Can anyone advise, is this as good as it gets or will it improve and does injections help ?? Thanks Greg.

I'm at an early stage in terms of this, and I'm no expert, but it might be possible to experiment with some different rings that cause your wife less discomfort?

Erections can continue improving for well over a year.
Do you get eny sense they are still improving?


When you say 4 Tadalafil tablets, that's presumably 4 x 5mg?


You could ask if you can try some 100mg Sildenafil instead - sometimes one works better than the other in different people.


As has been mentioned in other threads recently, a cock ring might help too. This is much more gentle than a pump constriction ring. However, it might well still feel uncomfortable for a partner.

Greg , I will never fully recover despite some promising early signs. I was supposedly non nerve sparring, so didn't expect to much. I am now nine and a half years on from surgery, 6 and a half years on from salvage RT and had two separate lots of SABR treatment. 


In the last few months I have been getting up with a respectable semi.


We had fun finding a solution and our orgasms have been out of this world. We mastered the technique of flaccid insertion. Never give up and have fun.

Our experience so far is to make sure both partners are shaved - getting the partners pubic hair caught in the ring is guaranteed to have you unable to continue due to laughing once they get over the pain.
The other advice is to not be too energetic - trying to get the maximum length inside will cause discomfort so try for shorter strokes.

Its been just over a year since I had a nerve sparing RP.


So errection wise things are slowly getting better. Not really good enough for intercourse, but ok for everything else. What I am trying to say is that everyone is different I am the same age as yourself and pretty fit. But still recovery seems slow. To be honest what I am really looking for on a daily basis is involentry errections and I have been getting a few more of those recently.


I use a pump daily, the DR also prescribed 100mg of sildenafil


If you can get a referral to a clinic then good to you, I have been referred twice in the last year but still never received an appointment. Services are patchy across the NHS.

Thanks for the info, I have heard Tadalafil doesn't help some people and maybe a change in tablet could help. I also have read 2 years can show improvement, and yes everyone is different. As for unexpected erections I have had a few mainly in the morning when busting for a pee lol. I does get me down but trying to stay positive. Thanks for your help.

I am taking this as a good sign. There is no intervention here, the penis is trying to stay healthy on its own without the blasted pump. The nerves are working to some extent.

Hi really sorry for not replying sooner i was interested in you saying you are prescribed 100mg of Tadalafil, I am taking 5mg daily, and 20mg before sex. I rang and spoke to specialist nurse on this site and got some really good information. I have also seen on NHS site it says you should not be on Tadalafil for longer than 12 months, I am going to talk to the Dr about this. Cheers Greg

The 100mg Tadalafil is a mistake. Max dose is 20mg.


Where did you find a reference to only being allowed Tadalafil for a year? That sounds wrong.

Came up on a Welsh NHS site from 2019 not sure if correct but iam checking with my Doctor. I have been on them for 12 months just 5mg a day and so far seem to of helped very well. The specialist nurse also said give it another year and hopefully maybe it will improve a little more fingers crossed. How are you getting on ?

I am 16mths since RP, erections are haphazard, and not useful. I recently got the hang of using the pump and together with a vibrating cock ring from amazon, it seems things are looking OK. However, my wife of late is on the cusp of menopause, so nothing in that dept for now. However, with the VED, I can get it up in a matter of seconds. It stays hard as well. My problem with the VED before was, the election was lost soon as I removed the pump. I don't know if it is a timing thing or I have healed.

Yeh I agree with what your saying sounds familiar to my situation even my wife is going through the MP so I have to consider her side of things aswell. I have used the pump and rolled the supplied silicone clear ring down on the penis and I seem to be getting the hang of things. I recently purchased some rings from love honey but just not tight enough they would of been fine before my OP. TBH I haven't had great success with taking the Tadalafil before sex doesn't seem to do much for me. I have found excitement and pump with ring works best. All the best Greg.
