Lutetium 177 failing

My husband diagnosed in 2008 and grateful of these years. Had 6 treatments of Lutetium 177 in 2022, last one December 2022, PSA 1 and bone mets zapped or reduced. PSA rising now 14.3. ALP 107. Oncologist only offering further chemo, but also referral to another oncologist for R223.

He suffers daily with Proctitis frequency and intermittent rectal bleeding following RT 2019, had endoscopy/colonoscopy, endoscopy diagnosed Barrett’s disease this explains difficulty in swallowing tabs, after insistence GP referred for treatment for Proctitis, awaiting appointment.

Abiraterone gave him 6 years of good QOL, have asked continually for Enzalutimide but say not eligible because had Abiraterone (3 months clause, inflexible protocol) Can get it cheaply from India but nobody to monitor, thought just trial for 3 months to see if any response.

 R223 seems the only immediate treatment then chemotherapy, unless someone on the community can suggest otherwise or an eligible trial.
