Worried about my fathers recent issues

Hi guys,

 

as title, im worried and confused about my fathers recent health issues and found this forum and looking for some experiance.

so last week my dad felt generally unwell and unable to pass urine.

he was admitted from the a and e, who found over 900ml of urine in the bladder. he had a catheter fitted to drain this and his symptoms improved.

on discharge he has a trial without catheter in 1 weeks time and the diagnosis was uti due to e.coli and a smooth enlarges prostate.

i checked his bloods results and psa was 54.

there has been no mention of cancer and he has a urologist app on march 24. what should i make of this.

posting an update to this.

 

so the first twoc failed and they repeated a week later which was classed as a pass.

he has his urology app in march.

 

hoping for the best case but he isnt feeling himself. urinating is very painful again after the catheter was removed and he just feels abit down since the whole thing.

will update as we go further

i plan on doing that. i have just ordered a psa test from medichecks. i know its only been a few weeks but it should indicate what way the number is going.

our family is visiting Pakistan and the end of this month and i have booked him a scan of the prostate and urology organs (sorry forgot the medical term they used) by one of the leading imaging hospitals at a cost of £110.

i know it might come across as overthinking and pushing too much, but i lost my mother to a very poorly diagnosed ovarian cancer and recently my dads brother to kidney cancer.

 

Edit: just been through the conversation with the imaging hospital. the scan is a PSMA CT scan which is apparently the latest innovation in prostate imaging.

I can understand with your family history why you want to get answers for your dad and I would be just the same. Hope everything is ok.

The scan that you mention PSMA CT scan, I assume this isn’t at a cost of £110? If it is we would all love to know where. I would double check as we paid for PSMA G68 around £5000.

Hi,

thank you everyone for your helpful replies. i have noted the advice on medichecks and will likely not use it again.

The Medichecks PSA came back at 14 which whilst still elevated, is a significant drop from 54. So atleast it has shown the higher number from the hospital to be a result of a different issue.

As for the psma test. it is being done at the PKLI centre in Lahore, Pakistan.

The pricing is correct, that is just the scan price and will be a few extra charges for the specialist interpretation etc but these are estimated at around £100 max.

i have checked the price against some other countries in the area and it is comparable to some highly regarded test centres in India.

 

After seeing your message regarding the PSMA i asked a familly member to pop in and they confirmed it is a PSMA scan with Gallium 68 as the contrast agent. 

Now i cant personally verify the company as this is my first visit to Pakistan at 33 years young, but have been assured by family members (dads brother and sister) who live there, that the hospital is a very highly regarded Renal and Liver centre.

When I had my PSMA scan done in the Uk the going rate was circa £2600. I read that they were doing it in Australia for what was said to be at cost at around £1000 at the time and I know it was even less in India but the price you have quoted is amazing. My scan was using 68 Gallium but I believe other ligands are now used too so this could have some bearing on the price.

Do let us know how it all goes and of course we hope that is well.

My cancer journey started via similar route, but that does not mean your father has cancer. The infection will cause a high PSA so that 54 is not a sign of serious cancer. A smooth prostate is not a sign of cancer. (Mine was knobbly)

However that high PSA from infection could be hiding a less high PSA from cancer, similar an enlarged smooth prostate from infection may be hiding an enlarged knobbly prostate from cancer so a bit of further investigation is to be considered.

It is reasonable that he should wait three months until all is settled down before further tests are done, because there is nothing to indicate cancer and his current symptoms would make further investigation for cancer impossible.

If the TWOC fails, he should request advice on self catheterization. Click on my profile picture, at the bottom of my biography are links to helpful threads, one is about self catheterization. Keep us informed, knowing whether this leads to a cancer diagnosis or an all clear will help future forum members access their own risk.

About two weeks prior to his urology appointment get another PSA test. I would just walk in to my GPs surgery and say I need a PSA test prior to my urology appointment. Some surgeries would want to see a letter from the consultant though.

Re the Medicheck pin prick PSA blood test, my experience of this on 2 occasions is that in my case it over estimated the PSA reading. In March 2018 the Medicheck reading was 6.26, in May 2018 having an injection blood sample taken by my Doctor the PSA reading came out at 3.58. When I again used the pinprick testing kit from Medicheck in Jan 2021 the reading came out at 7.54, a blood sample taken by the doctor in March 2021 gave a reading of 5.32. I was eventually diagnosed with prostate cancer and had my prostate removed late December 2021.

Hi guys,

sorry been away on holiday so not updated

First the scan was not done in pakistan due to troubles related to the general election at the time, was abit gutted but my dad's health was excellent at the time. no catheter, no urinary symptoms etc.

he had a psa test at the end of Feb and we met with the urologist last week.

this was a mixed appointment. PSA down to 5.2 which is a significant drop.

urologist performed a DRE as the original one was done by a general doctor on the ward.

 

DRE revealed a single small lump on the left side. right side feels fine.

Dad is scheduled for a biopsy on the 19th. skipping and MRI due to his pacemaker.

Urologist was excellant did make it clear that it is suspicious but also made a point to note that with the low PSA and what he can feel, his opinion is it that its something in its earlier stages.

now it's obviously not the best news but given my dad's 67 and we have had a strong history of very late stage cancers in the family, we are optimistic that if it is malignant that its early and has lots of treatment/curative options.

posting an update.

first biopsy was cancelled due to a uti. repeaf biopsy was done on 10th april. had a message today for an appointment with urologists on the 18th so quite a quick appoinment.

i hope im wrong but a appointment that fast leads me to believe were in for some bad news. 

will update after this appointment
