SABR treatment to pelvic lymph node

Hi just thought i would do a separate post to my story

I have my appt next week to discuss possible SABR treatment. In Nov a PSMA scan identified a 4mm lesion to one pelvic external iliac node. The oncologist has not ruled out SABR but has stated it would be technically challenging as the node is close to the previous RT field. 

Has anyone had SABR to a node close to the previous RT field? If so did it cause side effects?

Do you think its advisable asking for another scan as it was Nov when i had the last one. I don't want to have risky treatment if its spread further

Bri, looks like there aren't many of us on the same pathway. My first tumor was in an external illiac node, but there are apparently several external illiac nodes. My salvage RT was presumably  just to the prostate bed. When they found the second tumor there was talk of retreating the external illiac node again but I think they decided it was too risky to retreat it.

My PSMA scans were ten months apart and the second tumor was described by the radiologist as the smallest tumor she had ever treated.

Hi Chris,

Long time no see. A long story, which I will update on my personal story in due course.

I have a telephone consultation with yet another new oncologist tomorrow. I was due to have salvage RT over a year ago, but it didn't happen. See above.

My previous oncologist offered a potential SABR option for a secondary in a (presumed) lymph node in my "left pelvic sidewall". I'll discover what the new one thinks very soon. In the meantime I was wondering if you and/or Brian have any strong opinions on the pros and cons of SABR treatment?

My new oncologist turned out to be a very nice man with an even nicer virtual bedside manner.

In view of the recent reduction in my PSA level to 0.75 ug/L the current plan is for another test in 3 months time followed by a review of the situation.

Jim, sorry only just seen your post. For me the SABR treatment had no side effects, so I will happily have as many treatment courses as they want to throw at me. Unfortunately my last PSMA scan did not identify a definitive tumor so I may be metastatic and further SABR treatment may not be on the cards. 

If you get the chance of a PSMA scan and SABR the go for it. I have come to the conclusion that prostate cancer is all about guesswork and assumptions. If a PSMA scan picks up a tumor then hit it.

Thanks for the feedback Chris.

My personal circumstances preclude treatment just at the moment. The oncologist was talking about another PSMA scan after my next PSA, then SABR if no extra "glows" were visible.

I'm with you on the "guesswork and assumptions"!
