Hi

I am 65 year old

Had a recent MRI  PI-RADS 4 score

3 small lesion found on 75cc Prostate

DRE -Benign feeling prostate

0.5  PSA rise over 5 years to 4.3 now

Urologist is considering possible biopsy

Any comment would be greatly appreciated.

Definitely discuss need for Biopsy. My MRI showed a .9x.5x.9 lesion Pi-Rads4 and prompted the biopsy. It was ironic that the concern that prompted the biopsy turned out benign but the biopsy found on the same side a Gleason 7 (3+4) a Gleason 6 (3+3) and on the opposite side a Gleason 8 (4+4) and another 7. No indication on the MRI for any of the other.

Hello mate.

A warm welcome to you. I'm sorry you've needed to find us but glad you have.

Your prostate seems large, probably double the normal size of a man your age. There can be many prostate conditions that cause this enlargement and that could also account for your slightly high PSA.

To be on the safe side I'd be guided by your urologist, if he thinks a biopsy is appropriate, then perhaps it would be wise to follow his advice.

I believe PI RADS 4 means that the lesion is likely to be cancerous, so a biopsy would clarify your condition.

PSA had been jumping up and down last 18 months. Had been stable 1.9 for years, then jumped to 3.0, down to 2.4, up to 4.0, down to 3.3. DRE normal , Prostate volume 21cc

Also no treatment yet, awaiting PMSA Prostate Pet and Decipher genomic testing. I would like to take a year before starting any treatment.
