No, you don't need to worry. The result is telling you that your PSA reading is lower than they can measure ... it could be 0.000001 or it could be 0.019. Either way, it is at least 10 times lower than the level at which people would be talking about a recurrence & further treatment.

At 5 weeks, you've only had about ¼ of the dose so far. That result doesn't sound too bad to me, but I'm not familiar with what PSA profile looks like so soon. It takes about 200 days to get all the dose, but it is front-loaded, halving every 60 days.

However, PSA can continue dropping well after that, out to about 3 years max. You may get some PSA bounces in that time too.

Maybe the nurse has not seen many LDR brachy patients. If he normally sees RP patients then 4.5 would look very high 5 weeks after an op. Baring in mind that radiation doesn't kill cancer cells until they next divide and as Andy says you haven't had the full dose yet. I don't think you need to worry.

I'm assuming your not on HT. If you were then, yes it probably should be lower.

Apologies if I have picked the wrong place to post this. I had LDR brachytherapy 5 weeks ago, prior to the op my level was 11.1 and 4.5 this week. I was pleased but the nurse commented that 'it's not as low as we would hope' which gave me cause for thought. Is there a rough scale for lowering scores?
