Worried about increasing PSA

Not sure what I am doing wrong but tried to post a couple of times today, unsuccessfully - so here’s hoping for third time lucky! 

my OH has metastatic disease and has been on Prostap, following Chemotherapy until recently when his onco added Abiraterone as the Prostap was no longer effective.

these are his PSA results since starting the Abi

27/11/23 PSA 1.5

11/12  1.43

27/12 1.65

8/1 1.58

5/2 1.93

4/3 2.33

5/4 2.99

i know many of you on here are very knowledgeable and just wondering what your views are on the increase and the doubling time. Last spoke to the Oncologist following the 8/1 results and he was happy that it was stable and not increasing but that’s no longer the case.

Hi Mags, the rising PSA is not great news but my guess is that the onco will say you are coming to the stage where PSA isn't as important as how OH is feeling and what his quality of life is like. The metformin may help - there has been a recent trial which suggested metformin helps to control advanced PCa. See what the next couple of PSA results are like - if you have further rises and the April clinic doesn't happen, it might be worth paying for a private consultation with the onco just so you can ask questions about potential next steps.

hopefully your PSA will have returned to normal or at least be the same.

 

I I’ve read a few posts on here about inflammation causing a rise in PSA so perhaps it is that.

good that you are seeing the Urologist regularly anyway.

Your support is much appreciated. This really is a horrible illness when it’s not caught early and the constant round of test and wait for PSA results is hard for everyone, no matter where they are on this journey.

like a lot of people on here we get by by planning things to look forward to and that helps Rob and I to stay positive.

We have just had confirmation of our appointment on 20/4 so hopefully will get a few answers. 

I wish you well with your journey which sounds like it is going well - long may it continue.

Hi Mags,

 

I'm not sure what your OH means/stands for, but I'm 49-years old, and I'm also worried about my fluctuating/increasing PSA for the past 6-months:

Oct 4, 2023: Total=1.12, free PSA ratio=33%

Jan 31, 2024: Total=1.54, free PSA ratio=wasn't tested

Mar 5, 2024: Total=1.32, free PSA ratio=25%

Apr 3, 2024: Total=1.36, free PSA ratio=28%

Thats really helpful. QoL is not great right now and not sure if it’s just a case of bad luck as OH has been constantly unwell since Xmas, coughs, colds, chest infection, UTI etc and always very tired.

I think a private consultation might be the way to go as you just can’t beat seeing someone face to face.

Silly me - thanks for clarifying the meaning of OH, Mags :-)

I'm here because my total PSA jumped from its baseline of 0.75 (August 2022) to 1.53 (August 2023).

My specialist (urologist) suspected prostatitis, so he put me on antibiotics for 4-weeks, which brought down my total PSA to 1.12 (October 2023), but never returned to my normal levels (under 1). DRE was normal, but urologist said my prostate size has significantly increased in size over 12-month period. I'm expecting my next DRE to be next week, and he'll go over my recent PSA numbers from this year (2024) - and I'm super nervous from what he has to say.

As you've probably deduced, I don't fall in that category. However,  I would just like to say how sorry I am, that you are going through such a rough time. I hope something can be done to make life a little easier for you both.
