cyberknife - no visible cancer lesions

Hi ,

has anybody got any info / experience of on cyber- knife ?

do the cancer lesions need to be visible like for nano-knife and cryotherapy ?

my cancer lesions are not visible .,

does this rule cyber knife out for me ?

any advice / links  will be much appreciated

Bry ,I had SABR treatment to a couple of tumors the first one in 2022 and the second in 2023. I assume they need to see the tumor to treat it ,mine were picked up on PSMA scans. I was told the  delivery is super accurate, part way through treatment the physicist had to rewrite the program for the second tumor in 2023 too avoid damage to surrounding structures but still hit the tumor.

Not sure where all the experts are at the moment.

Radiotherapy is usually used to treat the whole prostate, so it isn't necessary to know exactly where the tumour is inside the prostate, although you do need to know how much it's necessary to treat outside the prostate. Focal radiotherapy in the prostate (treating just one or two parts) does exist (usually using brachytherapy), but isn't done in the UK as far as I know. A problem with it would be the difficulty in going back to treat another part of the prostate later.

If you are talking about mets outside the prostate, then they can't be treated unless they can be accurately found, with the exception of treating the prostate bed after a failed prostatectomy.

Bry, are your medical team not pushing you to make a decision. You appear to have been diagnosed eight months ago that seems a long time to go without intervention. You don't appear to be on HT, are you at risk of missing the boat.

Hi Chris and Andy,

thanks for your replies ,

my urology consultants are keen on surgery , i 'm not for "medical " reasons ,

i was hoping to have nano-knife ...whch left the door open for RT+ HT  if it didn't work ,

my prostate was 51ml , and i was prescribe avodart ,...

by the time i had a final MRI scan , my prostate had shrunk to 34mls , but my cancer lesions  , one positive ,2 negative  were not visible  , hence no nano-knife option .

i saw  oncology  last week at last , they say i can have RT ,without HT. ?

i guess this is because my prostate size ?

i don't know whether it's better to ask  on  have  HT , as well as a belt and braces ,

as RT is a one off chance treatmemt .

i appreicate what you are saying Chris , "active surveilance " is a gamble .
