Return of Erections?

I had nerve sparing  RARP 3rd November 2023. Had the usual ED issues – 60-70% at best now and Addenbrookes aware at last phone review. Wife is very understanding. Just this week I have been woken up twice by Nocturnal erections, very hard to almost painful! Maybe that’s what woke me! Not complaining, but its been a long time since I had  a stiffy!  It did go down quite quickly.  I can only get 60-70% “naturally”. Have not bothered with a pump.  Is this a sign thing are getting back to normal at last? Has anyone experienced the same? I am on 5 mg Tadalafil since a week after surgery to improve blood flow in that area. Looking forward to getting the same level of stiffness when  awake. No ED issues before surgery Aged 63. Thanks

Hi Graham.


I had non nerve sparing RARP, 18 months ago. I was told I'd never get a natural erection again and to date that has been the case. Weirdly the only time I get a semi is when I'm having a poo. Which is not an ideal situation to start foreplay. 😆


I think you've done very well to a have any sort of natural erection. I do get an almost immediate erection with Invicorp. I'd say it's about 85% as good as pre op. It lasts for two hours, which is much longer than I can.


I live in hope that one day things will return to normal but I'm not holding my breath.


I hope your natural erections continue to improve.

Hi Graham, 


You're definitely making progress. I had my operation in November 2019 and in July 2020, I was about where you are now. I'm now over four and a half years post op and things are pretty much back to normal. For me, recovery was gradual rather than overnight successes. 


I'd say keep trying and have lots of fun in the process. Maybe consider using a cock ring, some guys swear by them.

Hey Graham,


Agree with Kev. Its a gradual thing that we kinda just have to accept. I had my surgery in May 22 and was at least 15-18 months before something properly stirred again. I had 80% nerve sparing and was only 46 at diagnosis. In the last 3 months or so it has got significantly better. It will never be the same as before. I think most of us accept this.


However, I piss like a teenager and wont ever be suddenly told I have prostate cancer again. 


Maybe I have learned to embrace the small victories in life.

It's taken nearly 2 years since surgery for things to return to some level of normality. I used a pump regularly for all of that time but I am now getting to the point where I don't need it any more. I do however take 20mg of Tadalafil which helps things along for up to 48 hours.
