PSA of 3.6 -Holep in 2017

Hi, 

My husband had a HoLEP procedure back in 2017 for BPH. After this procedure his PSA was 2.8. All of his urinary symptoms cleared up after this procedure and he remains symptom free! 

He is now 56 and I have been on at him for the last 7 years to have his PSA tested annually and he just never scheduled a test…until a few weeks ago….he was having some blood tests anyway for an unrelated issue…(blood thinning medication) I asked him to request a PSA test and this result came back as 3.6…they will now carry out another and he’s just waiting to have this one done…I know regrowth of the prostate tissue after HoLEP is pretty rare so I can’t put his rise in PSA down to enlarged prostate this time…?

His PSA has risen over the last six years and now it’s above reference limit for his age….I am wondering if a raised PSA so many years after HoLEP (7 years) could be attributed to BPH again…or very unlikely as the HoLEP is supposed to reduce prostate size and it rarely grows back? We will speak with a GP obviously but at the minute the waiting is bringing back all the worry and trauma of 7 years ago and I can’t speak to him about my worry as I don’t want to worry him..so I find myself here amongst people who hopefully do t judge me for jumping straight into catastrophising. 🥲

Hi,  I knew nothing about HoLEP but looked it up.  My opinion, for what it's worth, is that prostates can get larger with age and as he still has a prostate it could be getting bigger.  It might not grow back after HoLEP but does that  stop the rest of the prostate getting bigger.

3.6 isn't that large,  a person of 60 has a limit of 4.   If they did a prostate measurement when he had his treatment perhaps they could measure it now.   All the best, Peter

Thank you so much for taking the time to reply Peter…..I do think what you’re saying makes sense and yes I have read that the prostate size can increase over time….even after HoLEP And that is what I will try to keep focussed on rather than slipping back down into the rabbit hole…I really appreciate your words.
