Got MRI results today

Got my multi parametric MRI results by telephone today. Abnormal areas in left part of prostate. Suspicious that it is cancer. It’s PI-RADS 5. Also some PI-RADS 2 in other parts of prostate. Nurse said it’s contained within prostate and no other apparent spread. My last PSA in June was 6.6. I’m 66 years old. I have a biopsy next Thursday 29/06/2024. Should I be worried. Is this curable? I have had prostatitis for over 25 years. I’m not looking forward to the perineal biopsy. Prostate is 37mm.

GP has failed to resolve the prostatitis for over 25 years, and not referred you to urology. That's not good (but nothing to do with prostate cancer). However, you now have the opportunity to speak to urology about it.

Welcome aboard Andy,

So your journey seems to mirror mine in a lot of ways. I started with PSA score of 4 back in June. My MRI results were similar toy your but only PI-RADS 5 and all in left section of prostate. I had the same biopsy as you have lined up and to be honest it was plain sailing (a bit embarrassing but easy enough). 

The biopsy will confirm the cancer diagnosis in the prostate if that is the case. To the best of my knowledge the MRI does not confirm the diagnosis but just that there is something there thats needs further investigation. 

If you go down the same path as I have been it will be a CT scan if the biopsy confirms cancer. This will show if it has spread. If this comes back clear then you may well have a PSMA-PET scan that will confirm any small amount of spread.

I'm currently waiting to see the surgeon to discuss if surgery or RT/HT is the best treatment plan for me.

As far as I'm concerned, the medical teams are still looking at this as a curative path and not just managing the disease.

It is very good new so far. Biopsy is never as bad as it sounds. Is it curable? - a million dollar question - if it is well contained a cure is definitely possible because you will probably have options of prostatectomy and targeted radiation options. I was lucky. I had prostatectomy at 72 and 12 years later I am still cancer free. Take time to choose an option. Since my experience there have been quantum leaps in all sorts of treatments. So you can look forward to a positive outcome. If you choose surgery I would be happy to help you in anyway I can; private message me if you like. A diagnosis of prostate cancer is a great shock and can be a lonely business; there is a lot of help here to chat. Good luck.

Antibiotics when it first flared up acutely out of the blue in 1998.,Having to go to the loo every 15 minutes. Several course of antibiotics over the years since then. Ciproxin being one of them. Years of chronic prostatitis since then. Just grin and bear it! GP was reluctant to just give antibiotics. I never knew if it was a UTI or prostatitis. Or what actual sort of prostatitis! Chronic Pelvic Floor Syndrome perhaps? I’ve had dysuria a lot, particularly the last couple of years. Really annoying. I have a slightly enlarged prostate too. I’ve had PSA checks every year since 2016. These being 5.3, 4.7, 4.8, 4.6,4.0, 4.8, 5.3, 6.3 (UTI at time of testing), 5.3, 6.6. The 6.6 was 13/06/2024. This prompted me being referred to Urology and the subsequent MRI scan.

It may be cancer Andy, IF it is, the biopsy will indicate how aggressive it is. Whatever  it's certainly seems to be curable. Good luck mate.

Can PI-RAD 5 on MRI turn out to be benign? Could it be infection/inflammation from years of prostatitis?

Prostatitis can raise PSA, and an MRI can't reliably tell the difference between prostatitis and cancer. Given where you are now, using the biopsy to resolve this seems sensible.

What treatment have you had for the prostatitis?
