Understanding the PET/CT Scan Report

I have been diagnosed with metastasised Prostate Cancer and I have received RT and getting HT.

Everything happened very quickly, from elevated PSA, MRI, Bone Scan, Biopsy, to PET/CT.

 

The PET Scan report states

"Corresponding to the same location at a lesion demonstrated on MRI there is focal PSMA expression in the left PZ involving the mid gland and apex with an SUV of 12.8.


Mild to moderate PSMA uptake in a 4 mm retroperitoneal left para-aortic lymph node at the level of L4 is indeterminate.  A lymph node metastasis could have this appearance, No other PSMA Avid lymph nodes. No enlarged lymph nodes,


A solitary PSMA expressing bone metastasis is present in the S4 sacral segment, this has a sclerotic appearance on
CT and an SUV of 18.3,


No other PSMA expressing nodal or metastatic disease identified.

 

I dont understand this:

"Mild to moderate PSMA uptake in a 4 mm retroperitoneal left para-aortic lymph node at the level of L4 is indeterminate.  A lymph node metastasis could have this appearance, No other PSMA Avid lymph nodes. No enlarged lymph nodes,"

Have you had an appointment to give you any of your diagnosis and to go through this result and explain treatment options?

I could guess lots of things based on this, but I don't have your full diagnosis or know what you've been told already.

Another key factor here is what radiotherapy treatments you can be offered. This is likely to need a specialist centre.

"left para-aortic lymph node at the level of L4" is simply referring to the location of the lymph node in question. L4 (from google) is apparently located in the lower region on the left side of the abdominal aorta. Your scan showed a "mild to moderate uptake" {of the PSMA tracer that was injected into you prior to your scan} in this lymph node. This tracer binds to PSMA protein (normally found in high amounts in prostate cancer cells) which helps "light up" prostate cancer in the scan. But it can sometimes bind to other tissue creating a false positive. From the sound of it, the individual(s) who reviewed the scan felt that based on the appearance in the scan, this small uptake of the tracer in this lymph node does not conclusively indicate that the node is cancerous. It's "indeterminate" as stated. Although PSMA PET scans are generally very accurate, false positives sometimes do occur. In the case of this particular lymph node, they are stating that they aren't sure. The good news is they also stated no other lymph nodes had any PSMA tracer uptake (meaning no cancer found). So you just have one lymph node that may be cancerous.
