Suitable treatments available for PNI and CRIBRI FORM PATTERN present in biopsy.

Hi everyone

Original info in my bio.

Well I received my biopsy results last week.

14 cores taken, 13 of which cancerous max length 10mm on one side and 12mm other side

Gleason 7 4+3=7 grade group 3 bilaterally(both sides of prostate)

Bilateral PNI and CRIBRIFORM PATTERN 4 present.

I'm now waiting for a PSMA PET CT to check for metastases.

I done some research regarding the PNI and cribriform and this is apparently a subtype more aggressive cancer.

The urologist has given 2 possible treatments given the biopsy results.

RP radical prostatectomy with SV removal or RT EBRT with ADT

Initial 3 months injection of DECAPEPTYL HT then 4 weeks of EBRT followed with up to 3 yrs of BICALUTEMIDE Tabs.

Just wondering if anyone has had similar results to me and what choices of treatment they were recommended.

My original PSA was 26ng/l  back in early July with a probable ECE T3a, 32 ccPV PSAD 0.08 also noted on mpMRI

No doubt these options could change once I've had scan but these were my options before he referred the scan after I raised the PNI and CRIBRIFORM mentioned in biopsy results.

I did have a conventional SPECTO choline tracer bone  scan back in July that showed no convincing spread to bone. I have since found out these scans were limited and not as accurate as the PSMA PET CT scans for finding micrometastases of prostate cancer elsewhere in the body.

Hi Dave

 I was Gleason 3+4=7, with PNI found in one core, and Cribriform in another. 11 out of 20 cores positive, bilateral disease.

I opted for surgery, which confirmed the figures above, although it also found EPE and Tertiary pattern 5 as well.

I had the surgery on July 17th, and am recovering well. Continence is still an issue but improving steadily. ED is a problem, as I was partial nerve sparing on one side only. Whilst this is frustrating, I consider it a small price to pay, as my first post op PSA test a couple of weeks ago came back undetectable.

The thought of surgery, is far worse than the actual reality.
