EBRT and brachytherapy start today

I start my radiotherapy this afternoon, I've had a last minute change to my plan on Monday to add a brachytherapy boost so now I'll have 15 sessions of EBRT rather than 20 then the brachytherapy on 19th Dec. It feels like the point of no return now, though I suppose really that was when I was diagnosed. I'm just worried about whether it's going to be successful and also the potential damage it will do to me in the process. Having said that, the doctors I've met have been brilliant and I feel I'm in safe hands.

Fingers crossed, and legs probably after having to fill my bladder up

I had that treatment in 2018. My most recent PSA was 0.1 . Of course everyone is different and past performance is no guarantee, but I've got nothing to complain about.

Firstly let me wish you a good recovery and hope this is the last time you will need any more RT 🀞and hope the HT time goes quickly.

Sounds very similar to what i,m having bar I have to have 3 months HT then the brachytherapy(24/1)before the RT (29/01).So what you’ve just written has given me some reassurance of things to come. Strange how they do things in different order 🀷‍♂️

Can I ask how you found travelling in everyday? Did you drive or do you need someone to drive you in?

I drove every day to my EBRT sessions with no issues at all, sometimes with my wife when she was free, but I always drove, I'm a bad passenger 🀣. I'm around 20-25mins from the hospital so not too far, I had read some people have had urgency issues but I think they were travelling further than me? Anyway, never felt concerned I wasn't going to make it in either direction.  The trickiest was finding a parking space, the NNUH has some spaces just for oncology which is nice.

For the brachytherapy my wife drove on the way there so she could just drop me off and not worry about finding a parking space. Then she collected me after, no driving for 24 hours after a general anaesthetic!

I found worrying before starting any of the treatment to be the worst, I guess fear of the unknown! Once started I was a lot more relaxed and in just became my routine for the 3 weeks.

All the best to you too, wishing you the same with a speedy recovery afterwards. I think us being a younger makes us a bit more resilient to the the side effects 🀞. Any more questions feel free to ask πŸ‘

As before really appreciate everything you’ve said. Sounds like my situation as well. I’m an awful passenger so was hoping I could drive in on my own or at worst just with my other half. Hope I get lucky with the timings as it can be 30 minutes if the traffic is good and 1 hr 20 in rush hour πŸ€¦β€β™‚οΈ Yep 🀞 it’ll be plain sailing or as much it can be. Really good luck for the next 18 months πŸ˜ŠπŸ‘

Hi don't worry about the Brachytherapy i had it in 2016 and it was an easy procedure at all.I was in and out of hospital with in two days and i am 8 years + and doing well and PSA below readable numbers they tell me.

thanks for the reply, I'm happier now the radiotherapy has started, I think it's fear of the unknown. Only had two so far but feeling ok. It feels weird that it can be doing anything at all to me, good or bad.

Still a bit nervous about the brachytherapy, but again I think fear of the unknown, though it looks very similar to the biopsy, hopefully it will be the last of the probing!

Is that at Mount Vernon? They do the HDR brachy after the external beam.

Though I'd update after my treatment, I had 15 sessions EBRT between 13th November and 3rd December. Had a go on all 5 of the LINACs the NNUH has to offer. It started to feel a bit like an odd version of Pokemon Go, I really wanted to see them all, but it was never guaranteed as the machine and times changed sometimes. Throughout though all the staff were lovely. I only had a couple of issues during treatment. On the second session I needed to drink more water as my bladder wasn't full enough, so had a couple more cups of water then wait 20mins before going back in. Then on the last session my bladder was too full and had to attempt to wee out a cup or so, estimating the amount was tricky! This last one was during lunch time, all the others had been in the evening, it seems I absorb water at different rates depending on time of day.

EBRT Side Effects:
Definitely became more wet farty! I'm sure the daily enemas didn't help, but never felt at risk of having any sort of accident and always felt in control. This seem to ease off after a couple of weeks and felt back to normal, just in time for the brachytherapy! I'm sure the rectal spacer helped in minimising these side effects.

After the first full week (I started treatment mid week) I was feel tired and dosed off very easily once back home, this improved from then on though.

Brachytherapy 19th December
At the NNUH this is normally done before the EBRT, I think mine ended up being done after due to the quite late change in my treatment plan which was going to be just 20 sessions of EBRT. The Brac was a day procedure done under general anaesthetic. I felt pretty relaxed about this in the end, I'd already met the Doctor as she also did my spacer. She was great and put me very much at ease, plus I was unconscious! The procedure went as planned, and was done in around 4hrs from what I remember! I was very much hoping to escape the hospital on the same day and without a catheter. I’d chatted with a chap during EBRT and he’d said he’d had to keep one in for 5 days, if I was the same that would have put me very close to Christmas Day which didn't appeal. Anyway after proving I could wee successfully without any fresh blood showing I got the all clear to go home around 3pm without the catheter. So I left with a 5 day course of antibiotics, some strong pain killers and some pills to protect my stomach lining from the pain killers. I skipped the pain killers in the end and protection pills as they didn't sound fun and I was never in any real pain, at most a dull ache.

Brachytherapy Side Effects:
Some blood in urine for a few days (similar to after biopsy) Again more wet farts which developed over the next week or so, still in control though. These are starting to ease off as I write this. The first couple of days after the procedure going for a wee was great, good flow and only up once in the night, I'm guessing the catheter helped open things up? This has gone downhill a bit, up a 2-3 times a night with reduce but not terrible flow. Was put on tamsulosin after the EBRT to combat this. During the day all good, it’s just night time that’s a bit of an issue at the moment.

It's the long term effects I'm more concerned about, only time will tell if and when they start to kick in, but for now I’m feeling good and a lot more positive than this time last year. I have a review in March, but I guess the real test will be when I'm off the Zoladex in 18 months’ time
