Worrying about ejaculation changes relating to BPH and PC

Hi,

So, I'm a 54 year old white man. Dad had PC (gleeson 9).  So I'm at elevated risk of PC and have had PSA checks every year since turning 50 - all have been below 1.

For the last couple of years I have been having some symptoms such as weak flow and dribbling.  I saw my GP a couple of years ago who performed a DRE and said it was normal, so I just carried on with life.

I became worried again a couple of months ago, so I saw my GP once again.  He did a DRE and said my prostate was moderately enlarged.  He prescribed tamsulosin which I've been taking for about a month.  I also had another PSA which was 0.8 so all good there.  The tamsulosin is definitely helping with my urinary symptoms. Would the tamsulosin affect my PSA score?

The thing that I'm worrying about is that over the last couple of years the volume of my ejaculation has reduced dramatically, and I can't work out if it's normal or not, or whether it can be linked to prostate changes.

I realise it can be normal for volume of ejaculate to reduce as a man gets older. What I have found is that instead of producing ejaculate at the start of an orgasm and on each 'pulse', I now only seem to produce ejaculate after 3 or 4 pulses have occurred and my orgasm is almost over. So, the first 3/4 pulses of the orgasm are 'dry' even though the sensations are completely normal.

I realise this can be a side effect of tamsulosin, however this was happening long before starting tamsulosin.  The tamsulosin hasn't changed my ejaculate much if at all.

Can anyone chip in with their experience of ejaculation changes as you get older?  I don't know if a reduction in volume looks as I have described above?  I imagined more that the volume would reduce evenly over the duration of an orgasm, rather than the delay I described?  I also thought that at 54 I should be too young to be worrying about this!  

I did ask the GP about the ejaculatory changes - he said it could be due to the enlarged prostate, but I can't find any literature to back this up - only descriptions of how meds for BPH can affect ejaculation.  None of the descriptions of the symptoms of BPH seem to mention ejaculatory changes which makes me worry that this ejaculatory change is down to something else?

My ejaculated sperm may have deteriorated prior to diagnosis but I didn't notice much. In hindsight my problem may have been the terminal dribbling. That started when I was fifty, I didn't need any night time toilet visits. My GP missed a PSA of 6.9 at 59 three years later a PSA of 7.7 prompted investigation and diagnosis. 

Many things can cause urine issues, sounds like you are being investigated and your PSA is quite low.
