Worried about MRI results

Hi everyone I’m new on here and felt the need to post something as worried but know answer will only come after biopsy.

in 2021 prostate found to be enlarged 85cc and PSA 8 Had a legion in transition zone rated PIRADS 3. Monitored.

2023 put on Finasteride and PSA fell to 3.3

Jan 2024 PSA rose to 5.11 and in Dec 2024 PSA was 6.7.

Aug 2024 had blood in wee pos damaged prostate by sitting on a rubber ring!!!

Late 2024 had normal full blood count, cystoscopy found prostate bled on contact, normal looking bladder, ultasound found normal kidneys and bowel ca test was negative.

3 days ago MRI (my first since 2021) Prostate size 90cc i’m guessing around 6cm x 6cm and the legion is now 4.3cm PIRADS 5. no lymphadenopathy and seminal vesicles normal.

Awaiting perinium biopsy originally local anesthetc but as i faint ive asked for a general. Urologist said in letter legion looks very suspicious so need to rule out changes are not due to cancer.

only thing is joint issues that flared up over last two years. Ie old mcl knee ligament playing up lead to sprained anckle on same leg and i have what gp thinks is tendonitis in both wrists that keeps coming back. You can guess what Im worrying about if i do have cancer.

not sure what I’m expecting back from writing this. Reasurance I hope as at the moment i’m thinking incurable ca, eventhough I should wait for biopsy before drawing any conclusion.

Prostate cancer doesn't go for joints or tendons, it likes bones and lymph nodes and is generally really quiet about it. With a PSA less than 10 you are highly unlikely to have a prostate cancer spread to your bones even if you do end up with a confirmed diagnosis.

Hello mate.

Sorry that you've had to join the club, but welcome to the forum.

Your prostate is about 3 times bigger than normal which can lead to raised PSA levels. Your MRI score of 5 means that there is a very high risk that its likely to be cancer. That's why you're having a follow up biopsy.  Only the biopsy results will show if it's cancer and how aggressive it's likely to be. We've had blokes on here with high pi-rads scores and found to have benign lesions. The great news is whatever you've got appears to be prostate confined.

IF the biopsy shows any cancer, it should be curable, and a treatment plan will be put in place to sort the problem out.

One of my biopsies was done under general anaesthetic and didn't feel a thing, so don't worry about that. Please keep us updated and good luck with your results mate.👍

As francij1 says none of us are medically experts, but there are a few on here that would get a decent score on Mastermind if they chose prostate cancer as their specialist subject. 🙂

Thanks for the welcome and for taking the time to put my mind at rest.

i know you can never be sure but it’s helped to ground me in the now and go out for a coastal walk with my wife rather than mope. So you’ve pleased her as well. 😁👍🏼

Hi there.

Had raised PSAs of 4.8 and 5.7 last year, MRI gave a '4' with no external signs (like yourself), just had a Truss biopsy (under general anaesthetic) and waiting on the results.

The Biopsy was fine - a bit sore and passing blood afterwards all normal occurances. Hope to be back on the bike in a week.

I can sympathise with your situation - after PSA and MRI it all seemed like a rollercoaster of symptoms, tests, negative possibilities and life altering treatment options - and what with Google doom scrolling I was really panicking!

Soon after a chat with a urology consultant was good and he suggested to take it all at one pro-active step at a time - which I'm trying to do. Even to extent that if it is PCa the MRI shows its not spread beyond the prostate and good options and positive outcomes are all in the majority.

Further trying to keep positive, have planned a 240mile gravel bike trek over 5 days and also signed up to a 100 miler flash mob ride to the sun overnighter in June. At 59 they'll likely to do me more damage than my prostate will ;-)

Good luck with your next tests - and getting out in the countryside meantime / having other things to look forward to really helps.

Thank you for replying that’s really helpful adice. Unfortunately my joints aren’t what they used to be but could still focus on other things i enjoy, like walking in the countryside and spending time with grandchildren and try not to give in to any gloom.

Just an update. Biopsy today and felt like a breeze and thats from someone who faints when coming around.

Appointment for results in two weeks.

So just an update

i had the results of my biopsy yesterday and  have no definite answer except that I do not have carcinoma.

They did though discover ‘some small lymphocytes’ and therefore there is a ‘suspiscioun’ of Lymphoma. It was also emphasised how rare this would be (0.01%)!!! 

So the biopsy samples have been sent to Lymphoma specialists to come up with a diagnosis and in the mean time i’m going to have a CT as if it is Lymphoma they’d need to know whether it’s in other areas.

Just an update I’ve had the MRI report back which showed the prostate lesion has spread to the other side of the transition zone but is still contained within the prostate capsule.No swollen lymph nodes nearby and no problems with the bony elements.

The biopsy Report came back saying that due to staining and small lymphocytes found there is a Suspicion of lymphoma but no carcinoma.

Samples were sent off for genetic molecular testing 4 weeks ago. I have had a CT scan that I’ve been told found everything normal except a 2cm nodule on my adrenal gland. I was basically told that the scan results indicated no Metastases.

hopefully I will know the outcome in four daystime but just wondered what peoples thoughts are. Based on what I’ve heard so far. Ihaven’t lost weight I don’t get fevers or have a skin rash. I’m able to walk for two hours but I do get the occasional night drench where I have to change the sheets sometimes it’s just a head sweat and I have to turn the pillow over but about 60% of the time I’m dry. I’ve also had night sweats on and off probably for about the last 10 years and put it down to the fact that I’m quite an anxious person although I gather also blood pressure tablets can cause night sweats

Anyway, it’s been just under two months since I had my MRI. It seems a lifetime of waiting and worrying but now hopefully I’ve only got four days left before all is revealed.

Just to update. I have been diagnosed with prostatic lymphoma. The Urology team are passing me over to the haematology team so I’ll find out more from them. But I’ve been told it’s a low-grade type of Non-Hodgkins lymphoma That should be very treatable sending the condition intermission but nearly always comes back. 0.03% chance of getting this kind of prostate cancer. Haven’t come across anyone else yet who has it!
