6 weeks post op review

Hi All

I will be shortly having my 6 weeks review following my Pc Surgery .Prior to surgery my Diagnosis was T3a and Gleason 8.

I am busy compiling some questions to ask the surgeon as I want to get as much information as possible.I know from reading a lot of the posts on the site that we have some very knowledgeable members so I was wondering if anyone could suggest important thinks to ask , such as do I have negative/positive margins etc.

I want to make the most of my time with the surgeon and don’t want to regret that I did not ask something particularly important whilst I had the chance.

Any suggestions would be greatly appreciated .

Ps

just looking after my 16 months old grandson ,I,m using more nappies than him at the moment 😂😂😂

But, hopefully, yours will only be wet John. 🙂

Joking apart the post op histology report contains lots of important detailed information. I'd ask for a full copy of the report, mate.

The obvious questions are what is my PSA now if taken? You want it ideally to be undetectable but could take longer than 6 weeks to achieve this. What was final Gleason Score? Do I have any positive margins? If positive margins are they focal or not? What is the size of positive margins? Do I have PNI or vascular invasion? Was there anything unusual reported by the pathologist? Were the seminal vesicles free of malignancy? If you do not know about nerve sparing you will want to ask were all nerves spared? If not what percentage of nerves were spared if any? Then any questions you have about incontinence and/ or ED. Will they prescribe now for ED and penile health? They might give you odds of BCR based on your staging, grading etc? May be good to ask if anything you can do now to aid full recovery.

To be fair the Urologist should answer most of these questions without asking. Finally ask for the pathologist report.

Good luck!

Have you thought about asking your doctor for a copy of the detailed pathology report? This will not only help you understand the surgical margins, but also the extent of the invasion, the tissue around the gland, and many other prognostic factors. I think having this will give you more information to ask your doctor or even get a second opinion if needed.

I think the previous answers have covered most questions to be asked. I had my surgery done in a NHS hospital we had some brilliant urology nurses who were at the end of the phone if needed. You may not understand all the technicalities of the answers , asking if you can record the conversation is an option.

Hope all goes well. Have you already had a PSA test, my hospital forgot to give me a form, so I went to the meeting not knowing what my PSA was. The recommendation is at least six weeks after surgery but as mentioned the PSA may not have fully reduced at six weeks.

Thanks Chris

Hi all

I was advised by my surgeon at the review that (thank god 🙏) the pathology report revealed that I had negative margins and my cancer was Grade 2 which he explained was good news.I have a blood test next week (12 weeks post surgery)and a meeting with him the following week where I will get my PSA result ,hopefully, this will be good news.I intend to ask him for a full copy of the Pathology report .I must admit I find all the different measures a bit confusing .My initial biopsy prior to surgery was Gleason 8 and T3a ,but the Surgeon at my 6 week post op review introduced this other measure of Grade 2 ,he explained that the Cancer was graded from 1 to 5 ,1 being slow growing 5 being aggressive ,but no mention of the Gleason score .I was just pleased with the news so intend to get further clarification at the face to face meeting in 2 weeks .Thanks everyone for your advice

Great news, mate. I believe, Grade 2 equates to Gleason 7 (3+4). Negative margins are very good. I always think it's a good idea to ask for a copy of results. I hope your post op PSA test is good. 👍

Please keep us updated.

Hello again mate.

Great news. 👍 If you don't mind me asking, what was your PSA reading?

He never actually gave a number but said it was not detectable or words to that affect .

i have asked him for a copy of the results of this and the pathology report on my prostate .

I asked about having access to my records and he said that I could and just needed to officially request them .

I think I have got something in a previous letter where I can request this .

i intend to update my profile with all the details .I hope to be able to compare the numbers in 15 years and not see any change 👍
