PSA drop of 1/3

I was diagnosed in January 2025 with Gleason 8 (4+4), T2N0M0.  

I am 54, otherwise well, and am scheduled for RALP next week.  

My surgery was postponed as the surgeon was ill (the irony) and I had my PSA redone this week to see if anything had changed.  

I got surprising results.  PSA levels:

Nov 24: 11.9 (this triggered referral and investigation)

Jan 25: 12.5

Mar 25: 8.68

I’m at a loss to understand what could have caused PSA to drop by almost 1/3 in less than two months.   Any insights would be welcome   I will of course be talking to the specialist nursing team as well

Did they put you on ADT whilst awaiting surgery? if so that could explain it.

Hello mate.

I'm sorry that you've had to join the club but welcome to the forum.

Presumably you've had no change of medication during the relevant time? It could be that at your initial test you may have had a urine infection or other prostate condition that caused a temporary higher reading. 

I'm sorry to hear that your surgery was cancelled. It happened to me twice. Both times, I was all gowned up and ready to go. The first time was for due to a sudden lack of beds. The second when the anaesthetist had concerns. It's horrible having to keep psyching yourself up for the op.

My husband had a PSA test which was 11.4 and a week later when seeing a private urologist he had another PSA test after a DRE examination and it was 9.2. That equates to almost a 20% reduction in a week. I think sometimes PSA just fluctuates. If you have had a biopsy which has provided you with a Gleason score 4-4 you are probably one of those men when it goes up and down. There are a number of variables that can make PSA fluctuate.

Could be.  I started insulin recently for type 2 diabetes so I wonder if that’s had an effect…

I didn’t get as far as you before op was cancelled and I’m sorry that happened.  

It could be medication, started insulin recently so wonder if it’s that.
