Blood in urine - what does this mean?

My father was diagnosed with localised prostate cancer Dec 2020. He completed radiotherapy in 2021 followed by 2 years of hormone treatment. He is in his late 70’s now. 
8 weeks ago he spotted blood in his urine. Very slightly. In the past week more blood is present. This week he collapsed at home as his iron levels became dangerously low. 
He is in hospital currently under bladder wash treatment. He is having his heart checked to ensure there is no lasting damage before they investigate bladder. Just wanted to know if this sounds similar to anyone. What the options may be so I can get a bit of an insight.

My blood and clots started a few weeks after salvage RT. I tried some hyperbaric oxygen treatment, unfortunately I think my severe radiation cystitis was diagnosed to late, make sure he is being treated by someone who understands radiation cystitis. It could of course be something different.
