Hi, I've just joined this community and trying to learn all the different terminology. I'm 69 years old and generally fit & healthy.

Briefly. back in April 2023 a routine blood test showed a PSA of 5.81. A retest in October 2023 a PSA of 6.27, then a further retest in April 2024 it was 5.13 - No further action.  Approx 1 year later, I asked for another test - 7th May 2025 my PSA was 7.14. Doctor called me in for DRE - prostate enlarged and referred me for MRI. A pity they didn't refer me a year earlier.

19th May MRI scan confirmed PIRADS 5. Biopsy now booked in for 11th June. Not looking forward to that having read a few horror stories!

It's not a good time, not knowing, fearing the worst and hoping for the best. Hardly any symptoms - a bit of urine "dribble" after a pee. I thought that was just a sign of old age. A few aches and pains around the small of the back and right hand side, could be muscular as I've had one or 2 back problems over the last few years, but every little ache and pain makes me think now.

Hi Dave.

I'm sorry that you're having prostate problems, but welcome to the forum mate.

Your PSA levels, although elevated, are not drastic. This could be caused by non cancerous prostate conditions including an enlarged prostate. Did your MRI give the size of your prostate and did it give a radiology cancer staging?

Pi rads 5, however, means there is a very high likelihood of cancer. Having said that, there have been a few on here with pi rads 5, that had benign tumours or no cancer at all.

I'm not medically trained, but with such low PSA it is unlikely that your general aches and pains are prostate cancer related.

Good luck with the biopsy results. I've had two and neither we're painful, just a bit uncomfortable.👍

Thanks for your reply. The only thing I know about the MRI is the Pirads 5, told to me by phone call from a urology nurse at the hospital. I haven't seen a report. Thanks for the reassurance regarding the biopsy. Much appreciated

Got a call from the hospital Wednesday saying they can do my biopsy earlier than booked, Thursday 05/06/2025.

So had my TP biopsy yesterday. I found the ultrasound probe particularly uncomfortable and some pain from the needle when applying the local anaesthetic. However, it wasn't nearly as bad as I feared after having read some comments online! Also must say the nurses and consultant surgeon were very friendly, respectful and kind.

They told me results of the biopsy take 2 to 3 weeks.  

I was given a copy of The Discharge Summary Notification that was being sent to my GP. This gave me further information about my Diagnosis that I wasn't aware of:

29cc prostate, DRE T2, Radiologically T3a PIRADS 5 lesion Lt posterolateral PZ. 

I guess that's not too bad. It looks like the cancer is localized and not spread, although T3a a bit of a worry.
