1st appointment after surgery

Hello I received my full pathology report a few weeks back after my robotic prostatectomy. This week I’ve got my first appointment since my operation. Below is my report 

acinar adenocarcinoma 

doninant nodule left side 30mm 3+4 =7 

smaller foci 22mm 3+4 =7 

extraprostatic extension - No 

bladder neck - No

seminal vesicles involved- No 

margin status involved 

extent - <3mm

gleason grade at margin 3 

involved location- apical, left side intraprostatic 

summary 3+4=7 

grade group 2 

stage pT2c NX 

can any one suggest any questions I need to ask my consultant when I see him this week? 
Can any one give any info or advice on the report? 
should I be pushing for salvage radiotherapy with having positive margins

amy one had a similar outcome after there operation.

Hello Gary.

The vast majority of the report is excellent news, the only slight downside is the positive margin. 

Your first post operation PSA test will be significant as it will indicate if the positive margin may be problematic.

Your consultant should explain things to you.

I'm not medically trained, but I don't think you should start worrying about salvage treatment at this time.

I'm glad you double checked, mate. Although they are only tiny numbers there is big difference between them, in relation to the undetectable aspect. Your .007 is an absolutely brilliant result. You should have a Martini, shaken not stirred, to celebrate Mr Bond. 😁

Try not to dwell too much on that positive margin, if your PSA does start to rise at some point, they'll know why, and where to fucus the treatment. 

I had a positive margin and had SRT almost three years ago, my PSA is now undetectable.

spoke with consultant at the hospital today, psa undetectable, sure the figure he gave  was 0.007. Asked about positive margin and was told not to worry to much while psa remains undetectable I’m no different to any one with negative margins. Only time they’ll act is if there’s a continuous jump in psa level. Thanks

Hi Gary, 

Thanks for posting. That is an excellent result mate. If he's told you your PSA is undetectable it means the assay you've used measures down to 0.007 which is very low. If that is the case there should be a less than arrow infront <0.007.

I trust no-one, mate and I'd double check the figure. My assays measure down to 0.02 and 0.04 respectively. Say yours measures down to similar levels and your PSA figure is actually 0.07 and not the James Bond .007, it would make it just above undetectable levels. Still a good result but not as good <undetectable

I've had wrong readings given to me in the past that's why I now always double check figures and look for that magic < arrow.

Whatever, I shall have a celebratory drink for you tonight. We're going through a good news spell. I love it!

Thanks I’ve double checked my results definitely 0.007 so feels like a bit of a weight lifted off my shoulders for now till I do it all again in 3 months time but for the moment I can file it away in back of my mind till the end of September. Thanks for your great response as usual. 👍
