It was an update from a recent conference so should be cutting edge...  The recommendations about 0.4 v 0.2 are exactly what I was told by the Royal Marsden 2 years ago.

 Mack Roach lll is terrific. I wonder if he's better than the the Mack I and Mack II editions. 🙂

I like his comment about urologists talking horse sh*t about radiotherapy when used as a primary treatment, limiting any salvage treatment. It's just a pity that opinion hasn't floated across 'the pond' to the UK. When my brother was diagnosed 7 years ago, and me two years later, numerous clinicians told us the same horse sh*t.

On a personal note, how are you doing, mate? I see there was a tiny rise in your PSA in December have you had any other checks since?

Hmmmn!. There are some differences between what even American based consultants say yet alone differences between those in America and this side of the pond. Effectiveness of HT prior to Radiation is a case in point. This doctor's opinion is more nuanced https://www.youtube.com/watch?v=cyY0nHXvzGc

Going back to the time I had my treatment, men were mostly given 3 months HT prior to Radiation but experience here had shown it beneficial to increase this from 3 to 6 months and this has become more widely adopted. I had this conversation with a top Consultant at the Royal Marsden because I was going to increase my HT to 8 months prior to RT. That was in 2008. Another contraversial statement is that it is untrue that black men are not more prone to getting PCa than light skinned men. All the information in the UK supports there is a difference. I would not take it that the views of American doctors are necessarily right and those in the UK are wrong. In fact the Americans lag behind the UK in several aspects, going over to Perineum biospy, widely adopting MRI before biopsy and introducing PSMA scanning as just three examples. Doctors can be like Politicians sighting whatever studies and statistics they feel appropriate to support thier differeng views, sometimes even using the same studies but with different interpretations. Don't forget we have NICE over here, a body of experts who give a lead. Having said that, as with other treatments Radiation continues to evolve and improve in hand with HT and other forms of treatment, with more personalised treatment for individuals.
