Increase in psa after robotic prostatectomy

I was diagnosed with prostate cancer and had my prostate removed 22/02/2024 (532 days ago) at the tender age of 46.


Gleason 3+4 and surgical margins were clear although by less than 1mm


My initial PSA was 0.025 but has risen to 0.12 last week.


I am now being referred to an oncologist and just waiting for the appointment.


Has anyone been through a similar situation.  For some reason I have found this more stressful than the initial diagnosis.

Hello, mate.


I'm sorry that you've had a post op rise. Your first reading was very low at 0.025, but was it ever deemed undetectable. Did it have a less than arrow < infront of it?

Thanks for the kind replies,  just checked original letter did have a < by and says undetectable.  Have my appointment  booked for the 20th.  Not sure why but this time I seem to always lean towards the worst case scenario.  


Golf lesson yesterday improved my mood (not my golf)

Hi,


I was in a similar situation and was referred back to oncology after three rises in my PSA. I had 20 fractions of salvage radiotherapy in 2022 and my PSA is now undetectable. 


I think that when you meet your oncologist, as others have said, you'll need to know about scans, if you need more PSA tests, and also, when it comes to treatment, will it include HT.

About 1/3 of prostatectomy patients will relapse like this. The oncology referral is normal practice.

Make sure they consider a PSMA PET scan as part of your restaging. Being so young it's important they get it all this time.

Hi, my journey seems very similar to yours. RP April 2024 and psa undetectable until around Jan 2025, then started to rise. At 0.2, I was referred to Velindre Cancer Hospital  and had an MRI of the pelvic area, but nothing could be seen. Two weeks ago PMSA PET scan done and results next Wednesday. I'm hoping it will show on the scan so they can target it. But consultant did say if they couldn't see it they may use experience and RT the prostate bed and start HT, not sure i really want HT!! . To be honest mentally it does feel worse than the initial diagnosis, as it feels like the last 15 months have been a waste of time and it's all starting over. However there's nothing I can do about it so just got to crack on and deal with it. Also signed consent form for knee replacement last week, so after 2 years on the waiting list, that's on hold until I know about the RT as I can't have both together due to healing. Like you, I'm a golfer, so my Thursday evening round is my saviour to forget about it all and go search for my ball in the woods 🤣🤣.
