2nd mri scan

I have just had my 2nd mri scan in the past 18 months. My 1st one was pi rads 2 and this one came back listed below. Does anyone know what it means? I'm now booked in for a biopsy at the month and very worried 😟.

MRI Multiparametric Prostate (Exam End: 20/7/2025 09:07)


 


Narrative


 


Exam: MRI Multiparametric Prostate


Indication: raised PSAD compared to previous MRI scan.


 


Multiparametric prostate protocol including diffusion-weighted and 


post-contrast Gadolinium enhanced sequences. Comparison: 19.1.24.


 


Findings: The prostate gland measures 4.3 x 3.8 x 3.6cm (estimated volume 


of 31mls).


 


There is a 10 mm focus of restriction of diffusion as a new finding within 


the left posterior mid gland peripheral zone (image 33, b1400). No 


accompanying hyperenhancement.


 


Stable intermediate T2 signal return from the peripheral zone. No 


concerning T2-weighted abnormalities noted within the transitional zone.


 


The prostate capsule and seminal vesicles return normal signal.


 


No pelvic lymphadenopathy.


 


Conclusions:


New PIRADS 3 focus within the left posterior mid gland peripheral zone.

Hi, mate.


A lot on the report is radiological guff. 


I see from previous posts, that your PSA in Jan 2024 was 4.7, in Nov 2024 it was 6.4. Have you had another more recent test? You are in your early 50's. Your prostate is a  normal size. There are other non cancerous prostate conditions and urinary infections that can cause a PSA rise but a steady rise over time is worthy of further tests.


The suspicious area on your MRI is pi-rads 3. This basically means they are not sure whether or not it is clinically significant cancer. The only way they can be sure is to do a biopsy. That will show IF it is cancer, and  IF it is, how aggressive it's likely to be.


The good news is whatever it is appears prostate confined.

Thanks for your reply.  I had a psa test Feb 25 came back as 5.1 then another in June 6.0.

So your PSA has remained relatively stable for over 18 months. That's good news. I'm not medically trained, mate, but the elevated levels and flucuations in your PSA may relate to prostatitis. Have doctors ruled this out as the cause?


I know you're worried, but honestly, your results, although worthy of further investigation, do not appear alarming. Get the biopsy done and get back to us with the results please. 👍

Prostatitis hasn't been discussed yet. But the more I look into it, it could be a possibility, as I have always have had the symptoms of Prostatitis on and off for years. I even had a ultra sound scan back in 2017 due to me keep having symptoms of uti's. Everything came back clear and I have since then I have just put it down to having a sensitive bladder and urethra from having really bad uti's when I was 17/18 years old. Peeing blood and very ill. Ending up having 4 cysoscopies. Finding that my urethra was very narrow, so they had to stretch it a few times. 


Either way im trying to find positives that if they do find cancer after my biopsy then I hope it has been caught very early and is treatable.

If you have not been tested for a UTI recently, it would seem advisable to do this. A culture rather than just a dip test is best..

If you do have some cancer, it would look from MRI to be contained, so could be radically treated with intent to cure, assuming there are no contraindications.
