Confused by treatment approaches and different messages

Husband Originally diagnosed 2019 - Hormone and RT treatment and clear for a few years - 2.5 years ago returned and had radical prostatectomy - all well for 18 months now PSA showing increase and metatastic cancer confirmed -low levels currently - originally saw Urology consultant who had conducted operation and was told 5 - 10 years maybe more - hormone treatment , then Enzalutmide - but then he saw oncologist last week who said 4-5 years and hormone and chemotherapy - results that I have seen from Enza look promising - so struggling to understand the change in treatment programme and prognosis - He is 64 year old relatively fit man in all other ways

Speak to the Onco, it may be he considers Enzo to be another form of hormone therapy (which it is).

If they don't answer your questions satisfactorily it's probably time to get a second opinion.

Over the past 10 years what used to be considered 2nd line hormone therapy (Enzo Abi etc etc) has in some cases become first line. Over the same time period Chemo has sometimes been deployed "up front" too. It really depends on the best fit for the patient and for this you are dependant on the skil of the Onco.

Unfortunately, there is a dearth of information on your Husband's Profile but if he has not had one, you could ask if a PSMA scan would be appropriate. If they can see only a few mets, it might be possible to treat these with radiation.
