Worried about my current urinary symptoms

Hello community, my story is: 


I am 40 years old no history of UTI nor prostate problems. 


Since May 18 2025 I had to go to emergency because of symptoms of UTI, I was prescribed 7days antibiotics . Got better but later in June had to come back again as i guess I infection was not cleared and ended up having epididymitis , bacteria was e.coli, after treatment felt better. But not 100% back to normal. 


Went to see urologist did all the tests, ultrasound, CT scan with contrast , cystoscopy all good but with the cystoscopy it could be seem inflammation , urethratis, according to doctor residual from hard infection I had , a part of that all look good. 


My PSA in May last year was 1.2, right after treatment for epididymitis was 10.6


I repeated it this week and was 1.6 


 To be sure as my symptoms persist but mild I had a MRI done. A well encapsulated nodule of about 2 cm was found in my 18 cm prostate in the transition zone. PIRAD was 3 with Likert 2 , adc of the nodule is 1.03. on the report it says everything bladder bone etc around is good and clear. 


I will have a biopsy done , I was told I could wait 3 to 6 months to repeat MRI but I decided Biopsy as I am very concerned about having cancer.


Just wanted to know if someone has gone trough a similar problem or would know a bit more about this. I am very worried. Just looking for opinions or help. 


I did rear that this nodules could be caused because of the epididymitis post healing process. I had blood rusty in the semen for few weeks in July but all good now. Looks back to normal.

It's possible that everything you have stems from the infection and it caused prostatitis (inflammation of the prostate). MRI isn't great at distinguishing that from prostate cancer. So given your concerns, probably only a biopsy is going to give you the certainty you're after.
