Change of treatment after 5yrs

First all thankyou for this forum we have read advice over the 5yrs since diagnosis.

My husband was diagnosed with Advanced PC in October 2020 aged 76yrs.

Gleason 4+5=9 T3b/4 N1 M1b prostate adenocarcinoma - PSA 240.

MRI prostate revealed abnormal obturator node and pelvic bony metastases and PI-RADS 5 prostate.  Multiple bony metastases in the pelvis Bone scan showed multiple pelvic metastases in the right symphysis pubis, inferior and superior pubic ramus, right ischium and left ischium matching muscle sclerosis.CT scan showed bilateral lung metastases largest at the time 22mm.

This is where it all began 5 yrs ago, so we set about doing research read leaflet after leaflet, googled everything we could on PC, but eventually we got ourselves into a routine of living our best lives and making memories and most importantly staying positive, taking about things when we need without it taking over and controlling everything we do.

Treatment has been 3 monthly Goresilin injection and daily Enzulutamide which  thankfully has kept his PC under control, there has been small rises in PSA lately on each blood test every 8 wks, last test a week ago PSA 0.54 and Testostrone level 0.9 so all reasonably good.

But the only down side to treatments the toll it has on the body, he's now had 2 heart attacks so his consultant has changed the hormone injection to Decapeptyl unsure if he's to stay on Enzulutamide as he's booked in for CT and Bone scan due to the rises in PSA consultant just wants to make sure Enzulutamide is still working effectively. He's not taking his Enzulutamide at present because of recent heart attack and its sensitively to another tablet he's been put on the same as his first heart attack 2 yrs ago. If Enzulutimide is still effective his dose will change to 2 tablets daily instead of 4 tablets for 1 Yr until he stops the other tablet again for his heart.

Just need advice on Decapeptyl injection and effectiveness as it's all very scary changing treatment when it's been working so well keeping his PC under control.

Thankyou

Oscar, combination of decapeptyl and apalutamide has brought my PSA down from 13.4 to 0.04 over the last six months.  I have had surgery and salvage RT plus a couple of pelvic lymph nodes treated.

Thanks Chris

Thank you Chris

Positive outcome to lowering your PSA level and may it continue for you Chris.

Hopefully Decapeptyl injection will work for my husband Dave. It just brings it all back starting a new treatment and then unsure if it will work, as I know we have been very lucky so far with Daves treatment and never taken anything for granted. He's never had surgery as it had already spread, & no RT.

He's got a CT scan and a Bone scan in the next few days than a face to face meeting with his consultant. To discuss the next steps if to continue Enzulutamide.

We hoping that the Enzu is still working and that he continues to take them, as worried to much change may not control his PC. But we will stay positive whatever happends.

Thank you Kim
