Is there anything after Abiraterone if you can’t have chemo?

My husband has had metastatic prostate cancer for at least 3 years. He has been in Abiraterone for about 18 months as well as Goserelin. PSA has been consistently very low at around 0.03 but latest was 0.8 and oncologist has ordered another CT scan and nuclear imagining which he has had this week to see if any progression. I know at some point the PSA will rise (hopefully very very slowly) but he has been told he is not suitable for chemo because of his heart. Are there any other options when we get a bit further along? He is 72.

Hi Linda, My OH was put on Radium (Ra-223) when Abiraterone didn't work. Ra-223 is usually well tolerated through 6 x 4 week injections and this has been the only treatment that we can't see any side effects. He has had two injections so far and we meet with the oncologist in between the injections. My profile shows the full journey. He had a PSMA PET scan before being offered Radium. His most recent PSA was 61.

Checking testosterone will confirm if the hormone therapy is working correctly.

The hormone treatment can be tweaked eg reintroduce Bicalutimide, they can also play around with steroids.

I assume his tostesterone level has been checked to make sure the existing regime is actually working?

We are not aware that his testosterone has been checked. Never been mentioned and haven’t seen it in NHS app.

I always get my Testosterone checked on my 4 monthly PSA blood tests.

he is on a steroid already (Prednisolone). We can check the testosterone level but not sure what that tells you? The nuclear imaging and CT scan should show any change but at some point the hormone therapy will start to not be so effective and  I was therefore wondering what else there is? At 72 and with hypertension and heart issues he is probably not a candidate for any clinical trials or am I being pessimistic?
