metastasis gone from bones and lymph nodes! we didn't know this was possible

We went for follow up this morning with Oncologist. Max had a nuclear imaging scan and CT scan a few weeks ago because his PSA was up slightly to 0.04. We were told that his bones and lymph glands show no evidence of disease now. It was originally in the spine and hip with extensive lymph node involvement in the pelvis.

He has been on Abiraterone and Goserelin for 2 years now. There is still cancer in the prostate but much reduced.  We were completely astounded (but delighted) because we understood that once prostate cancer spread it was incurable? We had hoped it would be reduced with the treatment but to completely disappear? Wow!

Just finding it hard to take in this amazing news. Has anyone else experienced anything like this? The treatment continues with 3 monthly PSA checks.  Has he just be incredibly lucky to respond to the treatment so well? Just hope he is not alone with his response and that it wont return?   His Gleason was 4 + 4 at diagnosis 2 years ago.

Hi my husband Gary was diagnosed 10 yrs ago ,Gleason 4+5 spread to lymph nodes. He had a course of chemo and then onto abi and enzyme combo on a trial. His PSA has been undetectable for 9.5 yrs ! He’s still taking abi and prosap .we know it’s not a cure but it’s a great feeling.

Congratulations on your positive result!

My early diagnosis showed likely metastases in some nearby lymph nodes and a rib. ADT (Zoladex and Enzalutamide) seems to have had a similar effect for me too - scans 8 months after starting treatment showed no evidence of active metastases.

Your profile makes no mention of radiotherapy. You may want to ask whether that is something worth having while the only active disease is in the prostate (I had this around 6 months after the start of ADT).

Yes, I am still on my 1st line treatment (HT) so am reasonably confident of beating the initial prognosis, without counting any chickens.

And reading the PSA results that Phil has achieved on the Halda trial really does bring hope.

The hormone therapy shuts down the cancer cells and reduces their size and proliferation. It's important to remember it's not a cure but a remission. When men respond well to the treatment the remission can last many years eg there are guys on here who are still in remission 10 years later.

We knew it wasn’t a cure but amazing that it has regressed so much! Reassuring that there are some men who have been in remission for many years. Can you stay on hormone therapy for years? We should have asked more questions but we were so taken aback that most of the cancer had gone. Next appt. Is a phone one so will have to write some questions down.

He had radiotherapy 18 months ago to the prostate. Told that would increase life expectancy of 3-5 years from 80% to 90%.  He is 3 years in already and now looking even more optimistic after these scans. what a great Christmas present!

Brilliant news Linda. Did the Oncologist say the metastatic spread has gone or shrunk after responding so well to treatment?

If it's gone does this mean your husbands staging can be reduced from T4?

Oncologist said no evidence of metastasis. We didn’t ask if that changed the staging but I thought once it was metastatic it was stage 4 and didn’t change? We will certainly ask at the next appt.

Hi Linda, yes I had similar results in April.

My mestasis' had completely disappeared back into the Prostate / groin area.

It's a great feeling, but my Onco was still not prepared to downgrade any of his initial diagnosis, I am still Stage 4 Incurable, with an outlook of 48-56 months and I am in month 48 now.

So good to hear that there are other men that are stage 4 are responding so well to treatment.  With so much shrinkage I would hope you would get quite a few years more than the prediction. There is hope with new treatments coming out that life expectancy can improve.
