Receiving Lutetium 177 Treatment

Hi All

I started Lutetium 177 Treatment back in September. Just thought I would post to try to help with an understanding.

 

I have received both Docetaxel and Cabazitaxel and neither brought any positive results. Please read my profile for more understanding on the progression.

 

Luckily I have private Health Insurance through my employer, so this made the treatment available.

 

14/08/2025 - PET Scan completed to understand if the body will accept the treatment.

18/08/2025 - PET scans positive so treatment to be arranged.

Six cycles to be administered every six weeks.

23/09/2025 - Cycle 1 - PSA 5.18, ALP 304

The treatment takes 2 hours to complete. The treatment is administered and then you receive a scan before leaving to ensure the treatment has been administered throughout the body.

Felt a little nausea during the first week after the treatment. A little off my appetite, by apart from that all was ok.

04/11/2025 - Cycle 2 - PSA 0.5, ALP 224 

No side effects after cycle 2

16/12/2025 - Cycle 3 - PSA 0.43, ALP 216

Again no side effects after treatment, consultant encouraged by the results from bloods taken.

Hello Parkie

I have had 177Lu treatment  ...  two doses -  first one 7.6 GBq, second one was 5,5 GBq  -  can you tell us how strong a dose you were given.  Also do you have a PET scan to visualise what is going on.   In my case I have been at psa <0.01 ng/ml for the last six months and the latest PET scan was clear.  Do you have any signs of 'dry mouth'?

Parkie, thanks for the update,info on the lesser used treatments is always useful. I was considered for lut177, but unfortunately a kidney issue meant it wasn't recommended.

Results and tolerance to it look good.Hope it continues to be if benefit going forward.

Hi Parkie,

Thanks for posting this. Very helpful for many of us. So glad to see that the side effects are minimal and 10-fold drop in PSA. Hope the benefits continue, and especially that you can enjoy this holiday season.

Hi Chris, sorry to hear the kidney condition prevented you receiving this treatment.

Hi Crispin, I haven't been advised the dosage that I received. I will enquire on that on my next treatment. As with chemotherapy I assumed it was administered due to size and weight.

Sounds like you have responded well to the treatment, have you just received two cycles and paused?

I will not receive a final PET Scan until after the last (6th) cycle. The consultant is going by the blood results currently and how I feel overall.

I have not had any problems with dry mouth or ulcers.

My next treatment is 27th January, will keep you updated guys.

Hello Neil

To answer your questions:  I had a PET scan before each dose of 177Lu-PSMA to see what was going on.  The treating Professor thought I would be back for a 3rd dose ...  but since my psa has been < 0.01 ng/ml and a PSMA-PET scan in November was clear -therefore I have had no 3rd dose.  However I have advised to contact the professor if my psa rises...

I am also taking enzalutamide -  this may be responsible for the psa value going down -  but this medication also makes the tumours more avid for PSMA (and this is just what is needed for 177Lu-PSMA treatment).
